Group Coaching Terms and Conditions
AGREEMENT:
Holly Turnbow (coach), of HET Coach & Consult Company, agrees to provide 8 weekly group coaching sessions starting Thursday, September 29, 2022 and ending Thursday, November 17, 2022. Group sessions will be held on Thursdays at 7:30pm CST. These sessions will be held on zoom and a link will be provided upon receipt of signed agreement and full payment.
Coach will also provide two separate individual sessions to each group participant (client) of the Group Coach Program. Client is responsible for scheduling individual sessions. The individual sessions must be conducted within the timeframe of the Group Coach Program and will each be 30 minutes in length.
Client will pay coach $147.00 via cash, check, venmo, or "Stripe accepted" credit card for these group and individual services.
After conclusion of the Group Coach Program, if continued coaching is desired, a new agreement will be drawn with new terms, sessions, and fees. Client may notify Holly Turnbow in writing to discontinue individual coaching at any time. Three month minimum commitment is required with all individual coaching packages offered by Holly Turnbow.
1. As a client, I understand and agree I am fully responsible for my well-being during my coaching calls, including my choices and decisions. I am aware that I can choose to discontinue coaching at any time. I recognize that coaching is not psychotherapy and that professional referrals will be given if needed.
2. I understand that “Life Coaching” is a relationship I have with my coach that is designed to facilitate the creation/development of personal, professional or business goals and to develop and carry out a strategy/plan for achieving those goals.
3. I understand that life coaching is a comprehensive process that may involve all areas of my life, including work, finances, health, relationships, education and recreation. I acknowledge that deciding how to handle these issues and implement my choices is exclusively my responsibility.
4. I understand life coaching does not treat mental disorders as defined by the American Psychiatric Association. I understand that life coaching is not a substitute for counseling, psychotherapy, mental health care or substance abuse treatment; and I will not use it in place of any form of therapy.
5. If I am currently in therapy or otherwise under the care of a mental health professional, I agree that I have consulted with this person regarding the advisability of working with a life coach and that this person is aware of my decision to proceed with the life coaching relationship.
6. I understand that information will be held as confidential unless I state otherwise, except required by law.
7. I understand that life coaching is not to be used in lieu of professional advice. I will seek professional guidance for legal, medical, financial, business, spiritual and other matters. I understand that all decisions in these areas are exclusively mine and I acknowledge that my decisions and my actions regarding them are my responsibility.
8. I give my permission for Holly Turnbow to keep a confidential record of my name, phone number and e-mail address in order to meet the requirements for coach credentialing by the International Coach Federation. I agree to let my coach document the coaching hours with me for submission to the ICF for certification as a coach.
9. I understand that group coaching is a joint venture between the facilitator (coach) and all group members. I am encouraged to participate in the process of developing and monitoring goals and outcomes. I agree to support the group, honoring and valuing each participant. Coach agrees to provide a fun and safe experience, encouraging members to share using I statements. Each participant makes the personal decision whether to receive feedback, and feedback is offered in positive terms without judgment or advice-giving.
10. I agree that all information shared in group coaching is confidential except in circumstances governed by law, such as child abuse and danger to self or others. All parties agree to take all reasonable measures to ensure confidentiality with any communication over the phone and/or Internet.
11. I understand that all fees for group are payable prior to the first group session to guarantee my place in the group and my commitment to the group and to receive the Zoom link to the sessions.
12. I understand that coaching is not considered medically necessary and is not covered by insurance. Cash, check, cashier’s check, and "Stripe accepted" credit cards are accepted. There is a $20 fee for returned checks. Fees are subject to change every six months and /or at the end of a group coaching program.
13. I understand that group sessions may not be rescheduled, and fees are non-refundable, unless Coach cancels or reschedules a group session. It is understood that I may not be able to make all of the group sessions, and it is requested that I let the Coach know as soon as possible when I will be unable to attend.
If the group member is under the age of 18, I attest that I have legal custody of this child and am therefore allowed to initiate and give consent for the child’s participation in these services.
ACKNOWLEDGMENT:
I have reviewed this Special Online Group Coach Program Agreement. I acknowledge that Holly Turnbow can make no guarantee or warranty as to the results of these services.
