PERMISSIONS
Please ensure you read and fill out all the sections of this form in order to ensure full knowledge and consent about the potential risks of the events that your child may be attending.

Transportation:
I/We, give permission for my child(ren) to be transported in a bus company contracted by Oliver Alliance Church or in a vehicle driven by a responsible adult leader over the age of 25, to the specified events at their respective locations. I understand that my child is expected to follow all applicable laws regarding riding in a bus/vehicle and is expected to follow the directions provided by the driver and/or other adult volunteers.

I have read, understand, and discussed with my student that:
1. They are expected to respect one another, the bus/vehicle they ride in, and the people they encounter while participating in Oliver Alliance Church events.
2. Riding in a bus may result in personal injuries or death, due to collisions, object interference, or the acts of those they ride with.
3. They will remain in their seat and not be disruptive to the driver of the bus/vehicle.

I recognize that by participating in this activity, as with any activity involving motor vehicle transportation, my child may risk personal injury or permanent loss. I hereby attest and verify that I have been advised of the potential risks, that I have full knowledge of the risks involved in this activity, and that I assume any expenses that may be incurred in the event of an accident, illness, or other incapacities, regardless of whether I have authorized such expenses.

Medical Information:
While every precaution is taken for the safety and good health, some sports and activities carry with them the inherent risk of personal injury beyond the risks associated with many of the recreational activities at Oliver Alliance Church. I/We understand and accept these risks and agree that by allowing my child to participate in those activities he/she may be taking part in a recreational activity that presents the potential for personal injury.

I/We, the parents or guardians named below, authorize the Director or one of Oliver Alliance Church Personnel to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant(s) named above.

General Waiver:
I/We, undertake and agree to indemnify and hold blameless Oliver Alliance Church, it’s Staff, Elders, and Personnel from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Oliver Alliance Church, as well as of any medical treatment authorized by the supervising individuals representing Oliver Alliance Church. This consent and authorization are effective only when participating in or traveling to events of Oliver Alliance Church. I hereby attest and verify that I have been advised of the potential risks, that I have full knowledge of the risks involved in this activity, and that I assume any expenses that may be incurred in the event of an accident, illness, or other incapacities, regardless of whether I have authorized such expenses. I also understand that membership at Oliver Alliance Church is not a requirement for participation in the aforementioned Oliver Alliance Church events.

Media Exposure:
I/We hereby grant permission for my child(ren) to be photographed and/or filmed without any reimbursement of any kind due to me, or the need to pay any fee. Oliver Alliance Church considers all information personal and confidential and will only use it within the church for ministry or other church-related activities and will not release it to outside parties or organizations.

Consent Clause:
I/We, understand that Oliver Alliance Church Staff will only be responsible for my children between allotted times of the event. It is the parent’s or legal guardian’s responsibility to ensure that their child arrives safely at the event, and is picked up on time after the event. Oliver Alliance Church will not be responsible for providing rides for anyone who is attending the event. Once the time of the event is over, the church will be locked and parents will be responsible for any children outside of the hours of the event and any actions they may do. Oliver Alliance Church will also not be responsible for any children who choose to leave during the hours of the event.

I/We have read and understood and agree with the above and sign it to cover all Oliver Alliance Church activities for the program year effective as stated below.

PLEASE NOTE: During an event, if someone wishes to leave without our knowledge, this would be possible. All students must be responsible enough to listen to leaders and stay on the premise for the duration of the event. We will not be responsible if students leave without our consent. Please have this conversation with your student(s). You are responsible to drop off and pick your student(s) or to have a plan for their arrival to and from the event.
Though we will never be able to guarantee the supervision of students at all times for an event, we always encourage a buddy system and have leaders roaming the area keeping their eyes open. Your child(ren) need to be responsible, looking out for their safety and the safety of others, listening to instructions from the leaders and staff, and encouraging communication. We expect our student(s) to act with maturity and safety. We will create the safest environment possible during all of our events, but accidents happen and unforeseen things do occur. As well, we expect students to be responsible enough to keep everyone safe and follow the rules.

Oliver Alliance Church considers all personal information as confidential and will not release it to outside parties or organizations. However, by completing this form you are providing consent for this information to be used within the church for ministry and other church-related activities. When using photos of Oliver Alliance Church events, either on our website or on OAC youth social media sites, we try to post photos that would not be considered objectionable. We will remove any photo immediately upon request.
