YES! I want to participate in the Gridiron Football JR All-American Bowl Game to be played on December 30, 2022 in Orange Beach. Check-in is December 27th 2022 from 9 to 11 am. I am enclosing my $300 fee/deposit and I under- stand that this will save me a spot on the 2022 All-American Team and it will pay for the expenses incurred for the Bowl Game. I also understand that all of my expenses for the 2022  All-American Bowl Game will be paid for, once I get to Orange Beach. Also, I agree that if I cannot participate, because of a legit injury, I will give a 30-day notice before the game and will be refunded my $200 of the fee/deposit. I also understand that if I fail to show up to play in the 2022 Gridiron Football JR All-Americn Bowl Game without prior notice, my $300 fee/deposit will be forfeited. Additionally, if I understand that if I am asked to leave due to non-compliance with the Gridiron Football Jr All-American Bowl Game rules and/or hotel rules my deposit/fee will be forfeit. I understand that I am responsible for my room. This packet, along with your payment of $300 needs to be submitted by Novemebr 1st, 2022. Any payments re- ceived after Novemebr 1st, 2022, there will be a $50 late fee.

GUARDIAN OF TO PARTICIPATE IN THE GRIDIRON FOOTBLL JR ALL-AMERICAN BOWL GAME I HEREBY RELEASE THE GRIDIRON FOOTBALL ALL-AMERICAN BOWL GAME,  GRIDIRON FOOTBALL, ANY OFFICALS, OR ANY OF THE DIRECTORS OR COACHES FROM ANY LIABILITY AS A RESULT OF ANY INJURIES WHICH MAY OCCUR DURING MY PARTICIPATION. I FULLY UNDER- STAND THAT I AM RESPONSIBLE FOR ANY ALL MEDICAL EXPENCES THAT MAY BE INCURRED AS A RESULT OF ANY ACCIDENTAL INJURIES. I FURTHER UNDERSTAND THAT THE GRIDIRON FOOTBALL ALL-AMERICAN BOWL GAME IS A FOUNDATION AND ASSOCIATION.

I hereby grant Louisiana Gridiron Football LLC, Gridiron Football the irrevocable right and permission to use photographs and/ or video recordings of me website and in publications, promotional fliers, educational materials, derivative works, or for any other similar purpose without compensation to me.

I understand and agree that such photographs and/or video recordings of me may be placed on the Internet.I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast infor- mation that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any repro- ductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of Louisiana Gridiron Football LLC.

I hereby release, acquit and forever discharge Louisiana Gridiron Football LLC, its current and former trustees, agents, officers and employees of the above-named entities from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assigns and personal representatives.

If individual photographed/recorded is under eighteen (18) years old, the following section must be completed: I have read and I understand this document. I understand and agree that it is binding on me, my child (named above), our heirs, assigns and personal representatives. I acknowledge that I am eighteen (18) years old or more and that I am the parent or guardian of the child named above.
