YES! I want to participate in the Gridiron Football JR All-American Bowl Game to be played on December 30, 2022 in Orange Beach. Check-in is December 27th 2022 from 9 to 11 am. I am enclosing my $300 fee/deposit and I under- stand that this will save me a spot on the 2022 All-American Team and it will pay for the expenses incurred for the Bowl Game. I also understand that all of my expenses for the 2022 All-American Bowl Game will be paid for, once I get to Orange Beach. Also, I agree that if I cannot participate, because of a legit injury, I will give a 30-day notice before the game and will be refunded my $200 of the fee/deposit. I also understand that if I fail to show up to play in the 2022 Gridiron Football JR All-Americn Bowl Game without prior notice, my $300 fee/deposit will be forfeited. Additionally, if I understand that if I am asked to leave due to non-compliance with the Gridiron Football Jr All-American Bowl Game rules and/or hotel rules my deposit/fee will be forfeit. I understand that I am responsible for my room. This packet, along with your payment of $300 needs to be submitted by Novemebr 1st, 2022. Any payments re- ceived after Novemebr 1st, 2022, there will be a $50 late fee.

GUARDIAN OF TO PARTICIPATE IN THE GRIDIRON FOOTBLL JR ALL-AMERICAN BOWL GAME I HEREBY RELEASE THE GRIDIRON FOOTBALL ALL-AMERICAN BOWL GAME, GRIDIRON FOOTBALL, ANY OFFICALS, OR ANY OF THE DIRECTORS OR COACHES FROM ANY LIABILITY AS A RESULT OF ANY INJURIES WHICH MAY OCCUR DURING MY PARTICIPATION. I FULLY UNDER- STAND THAT I AM RESPONSIBLE FOR ANY ALL MEDICAL EXPENCES THAT MAY BE INCURRED AS A RESULT OF ANY ACCIDENTAL INJURIES. I FURTHER UNDERSTAND THAT THE GRIDIRON FOOTBALL ALL-AMERICAN BOWL GAME IS A FOUNDATION AND ASSOCIATION.
