In consideration of being permitted to participate in the activities at Clearfork Creek Farm Corporation (*CCFC"), First Name* Last Name* (the "Participant") agrees to the following:
Participant releases, waives all claims against, discharges and covenants not to sue the CCFC Board of Directors, Garden Manager, volunteers, members, or representatives (collectively referred to herein as "Garden Representatives") with respect to Participant's activities with CCFC, including without limitation attendance at, observation of, or being otherwise involved in the activities (the "Participation"), whether my claim or the claim of anyone on Participant's behalf is based on or arises out of personal injury, death or injury to property and whether such claim is caused by the negligence of the Garden Representatives.

Participant assumes full responsibility for and risk of property damage, bodily injury or death due to the negligence of the Garden Representatives with respect to Participation within CCFC.

Participant agrees to indemnify and hold the Garden Representatives harmless from the claims of any third party arising as a result of my Participation in the activities of CCFC.

Participant gives permission to the Garden Representatives to release to the media any photographs taken of individual volunteers or a group.

Participant has read this waiver in its entirety and understands that by signing in today, Participant is releasing all future claims Participant may have against the Garden Representatives with respect to any Participation in the activities of CCFC.
