  • Section A: General Information

    For the purposes of this document, HolisticalVets LLC and any its veterinarians, technicians, assistants, and other employees will hereafter be collectively referred to as HolisticalVets LLC. Please be aware the HolisticalVets LLC is a separate entity from any other place HolisticalVets LLC's employees may work.


    To initiate an appointment request for your pet, we request that you complete, sign, and submit this Patient Consent Form and request that your pet's primary veterinarian/veterinary facility send your pet's medical records directly to info@holisticalvets.com (as only medical records received from a veterinarian will be accepted). Upon receiving your completed Patient Consent Form and pet’s medical records, our team will review the information to determine how we can best assist you and your pet. If we determine that we can be of assistance, we will reach out to you via email to express our interest in offering our services to your pet. This email will include a link to our appointment scheduler, allowing you to conveniently schedule an appointment and you will have the option to request an individual appointment or a package containing either four (4) or six (6) appointments. Please be aware that completing and submitting this Patient Consent Form and authorizing your pet’s medical records be sent to HolisticalVets LLC does not guarantee that HolisticalVets LLC will be able to provide services to you and your pet.


    HolisticalVets LLC's primary mode of communication is via email. Rarely, HolisticalVets LLC may text from 848-276-7238 when needing to communicate with a client regarding a confirmed appointment scheduled that same day. As calls/texts to 848-276-7238 may not be monitored at all times, any communication (other than responding to texts that HolisticalVets LLC initiated) should be sent in the form of an email to info@holisticalvets.com. Messages sent through social media accounts or to other email addresses are not routinely monitored and HolisticalVets LLC is unable to accept any information/questions regarding pet concerns, medical questions, and appointments, etc. to any form of communication other than email to info@holisticalvets.com. 


    HolisticalVets LLC's business hours can be found at the bottom of this page (if you are viewing this form while on HolisticalVets LLC's website). Please be aware that these days/hours are subject to change without notice. 

    Please note: Due to a large service area, there is a 2 hour (120 minutes) estimated time of arrival window for HolisticalVets LLC to arrive at the designated appointment location.


  • Section B: Services

    HolisticalVets LLC only provides services limited to in-home euthanasia and certain holistic modalities and does NOT provide any emergency services. HolisticalVets LLC also does not provide conventional medicine services (except for in-home euthanasia services and the occasional sedative if needed for treatments). Please click here for a list of veterinary emergency and urgent care facilities in the counties we service. 

    Since holistic medicine is not a substitute for conventional/Western medicine, HolisticalVets LLC requires all clients to have a primary veterinarian that provides conventional/Western services. HolisticalVets LLC firmly believes that an integrative approach, where both conventional/Western and holistic/Eastern medicine are utilized, offers the best care for animals and there may be times where HolisticalVets LLC recommends conventional/Western diagnostics and treatment for your pet. 


    Information about the service(s) HolisticalVets LLC is currently offering or may offer in the future can be found here. Oftentimes, holistic medicine and treatments require more time and effort from everyone involved and since holistic medicine is not a "quick fix", overnight resolution is uncommon. Depending on the modality used, most conditions need 4-6 treatments before the effectiveness of the treatment can be accurately assessed (even though it can be common for pet owners to notice an improvement after the first treatment). While conventional or Western medicine may provide faster results, holistic medicine aims to address the underlying causes of health issues rather than just treating the isolated symptoms, resulting in a more comprehensive and long-lasting solution. Regardless whether holistic or conventional medicine is used, every animal is unique and may or may not respond to treatment in different ways, as there are no guarantees in either type of medicine. 

    Holistic medicine can be helpful for a multitude of problems that may afflict pets. These problems are not just limited to diseases and illnesses affecting muscles, bones, and nerves, but also those that affect behavior, the skin and other organs, the immune system, and so much more. While some conditions may be able to be resolved completely, other times (just like in Western medicine) there are afflictions that cannot be resolved (or if they can be, only partially). Oftentimes, especially if the condition is chronic or at or near end-stage, the goal is to simply extend the pet's quality of life and to slow down or possibly halt the progression of the disease/illness for as long as feasibly possible.

    HolisticalVets LLC welcomes questions regarding your pet's exam findings and treatment recommendations, as it is important that you have information to make decisions for your pet that you feel comfortable with. Please be aware that while internet searches can prove invaluable at times, there is much misinformation about veterinary medicine on the internet and that internet searches do not take the place of years of schooling and advanced hands-on training.


    HolisticalVets LLC requires that your pet be up to date on their rabies vaccination prior to any appointments for holistic services if a licensed rabies vaccine is available for your pet's species and age (unless your pet's medical records state that your pet should not be vaccinated for rabies). 


  • Section C: Treatment

    HolisticalVets LLC's services are limited to certain treatment modalities and charges for services are not dependent on the treatment modality used, but instead are based on the number of appointments scheduled. Charges were designed in this way as to not let finances dictate which appropriate and indicated treatment modalities are used. Please be aware that anything outside the scope of what's included in an appointment will incur additional charges.     Treatment recommendations for pets are made as deemed appropriate and indicated by exam findings, medical history, and other applicable information. In the case that HolisticalVets LLC finds that a pet may benefit from modalities and diagnostic and treatment options not offered by HolisticalVets LLC, clients will be advised to seek out those options elsewhere. Clients have the option to decline HolisticalVets LLC's recommendations for their pet; however, it is with the understanding that when a client declines such recommendations, the effectiveness of their pet's treatment may be partially, or even fully, reduced. Even if treatment has already started, clients have the option to stop treatment on their pet at any time. 


    If food therapy services are sought, please be aware that, while certified veterinary food therapists (CVFTs) are veterinarians that have completed additional nutritional education, training, and testing in order to be able to appropriately and correctly formulate nutritionally balanced diets and recipes for animals, boarded veterinary nutritionists have undergone far more training and education in nutrition than CVFTs; therefore, it is always recommended that clients consult a boarded veterinary nutritionist first and foremost if interested in having a diet specifically formulated for their pet(s). 

    For pets following food therapy recipes (which are tailored to address the specific diagnoses and/or unique needs of those pets), regular examinations by a qualified veterinarian trained in food therapy are crucial. These check-ups play a pivotal role in monitoring the health of those pets and ensuring the continued effectiveness and safety of those diets while making any necessary adjustments.


    If herbal therapy is utilized, please be aware that several different herbs may be trialed to see which one(s) work best for your pet and that they may change based on your pet's current and/or changing needs. In addition, regular examinations by a qualified veterinarian trained in herbal therapy are crucial. These check-ups play a pivotal role in monitoring the health of your pet and ensuring the continued effectiveness and safety of those herbs while making any necessary adjustments.


  • Section D: Reducing Patient Fear/Anxiety/Stress

    HolisticalVets LLC practices Fear Free techniques in all interactions with pets, as it firmly believes that animals should feel safe and be treated with kindness, respect, compassion, and patience. 

    HolisticalVets LLC will always make every effort to help reduce any fear, anxiety, or stress a pet may feel; however, means of protection (e.g., a muzzle, swaddling, etc.) may be indicated to help protect the pet, client, and staff. Please be aware that fear/anxiety/stress and/or use of a sedative may decrease the effectiveness of treatments provided. 

    HolisticalVets LLC requests that clients do not physically hold down their pets while the pet is being examined or treated, as client and pet safety is extremely important to HolisticalVets LLC. If a pet's fear/anxiety/stress is present, HolisticalVets LLC requests that clients distract their pets with treats instead of restraint for the duration of the examination(s) and treatment. 

    If a pet shows signs of aggression, severe fear/anxiety/stress and/or doesn’t allow for HolisticalVets LLC to provide services, HolisticalVets LLC will not force the pet to do something he or she is not comfortable with (as excessive fear/anxiety/stress can cause serious, and sometimes fatal, consequences) and payment for that particular appointment is non-refundable/non-transferable. Another appointment may be scheduled and attempted if HolisticalVets LLC deems it's safe to do so for all parties involved and use of sedation may be recommended for that rescheduled appointment, with the understanding that the rescheduled appointment will incur its own fee(s). 

    If there are any other household animals that have the potential to exhibit aggression towards people or animals, HolisticalVets LLC requires that they be securely confined away from HolisticalVets LLC’s staff at all times.

    HolisticalVets LLC requires that scheduled pets be easily accessible and available in a safe secure room/enclosure at the start of and for the duration of the scheduled appointment time to enable HolisticalVets LLC to provide services. The safe secure room/enclosure should be stress free and not offer the pet a hiding place that is difficult for humans to reach. If the pet is not easily accessible and available, the client is responsible for extracting the pet out of the hiding place. If the pet is not easily accessible and available for the scheduled appointment, HolisticalVets LLC reserves the right to end the appointment and any payment for that appointment will be non-refundable/non-transferable.


  • Section E: Precautions and Contraindications

    Please note: HolisticalVet LLC performed extensive research to formulate the following list of precautions and contraindications; however, it is with the understanding that this may not be an exhaustive list. Should HolisticalVets LLC become aware of other precautions and contraindications that are not listed below, an updated list can be found here. 


    De-Qi response: One of the goals of acupuncture is the arrival of De-Qi, which is a sign that treatment is effective. Animals will sometimes react to De-Qi by yawning, passing gas, flicking their ears, etc. Other times, the reaction may be vastly more pronouced and sudden (e.g., a horse suddenly kicking or jumping). Regardless of the type of response, it can last anywhere from 10-30 seconds and then the animal will resume relaxing. While the arrival of De-Qi is always a good and wanted thing, the concern is if the response to De-Qi causes a secondary injury (e.g., a horse suddenly and unexpectedly kicking a stall door, hurting himself or herself or someone else); therefore, we recommend that animals larger than a dog be in a large and open space for treatment without any other animals close by. If people are nearby, we ask that caution and spatial awareness be utilized to minimize the risk of injury. Animals that are extremely weak or have neurological diseases are less likely to exhibit a pronounced response to De-Qi. 

    Dirty/wet haircoat: To minimize the risk of infection, HolisticalVets LLC is unable to perform acupuncture on pets with dirty or wet hair/fur. Please ensure that scheduled pets have clean and dry hair/fur for scheduled appointments (this does not apply to pets that do not have hair or fur).

    Acupuncture treatment duration: Please be aware, for the purposes of acupuncture, the goal is not to insert as many needles into the body as possible during an appointment. Instead, the goal is to select one to ten plus key points that provide multiple benefits and are indicated for a pet's diagnosis and treatment goals. Often, fewer needles will be used in the first few appointments to give the pet time to get used to acupuncture and over time, a pet will usually come to expect acupuncture when an appointment begins. Needles are then removed when a pet indicates that he or she is ready to end that day's treatment. For pets that will happily enjoy acupuncture for hours on end, acupuncture needles are usually removed after 30 to 50 minutes. 

    Pregnancy/Nursing: HolisticalVets LLC should be informed if there is any chance the scheduled pet is pregnant or lactating, as there are some acupressure/acupuncture points and herbs that must be avoided in pregnant and nursing animals.

    Meals/Exercise: Appointments should not be scheduled less than 1 hour after meals and/or exercise, as the the scheduled pet's body needs time to digest and rest prior to treatments. It is common for the pet to be tired, sleepy, or calm after a treatment and he or she should be allowed to rest (minimal to no physical activity) for the next 24-48 hours and food should not be given to the pet in the hour following treatment. A pet may seem sleepy, tired, or even worse 24-48 hours after a treatment, which is normal due to the body needing time to reset. 

    Competition: For pets undergoing herbal therapy, herbs should not be given for 7 days prior to any competitions.

    Pigmented urine: Pets that receive Vitamin B12 may have discolored urine (e.g., pink, red, bright yellow, orange, etc.). This occurs when the kidneys excrete excess Vitamin B12 after the body absorbs what it needs. 

    Illness/disease and medical devices: HolisticalVets LLC should be informed if the scheduled pet has an illness or disease (e.g., heart disease, seizures, cancer or tumors, etc.) or a medical device (e.g., pacemaker). 

    End of life care: Acupuncture/acupressure can be used in end of life care, with the understanding that animals near death may improve temporarily with treatment but pass away shortly after. 

    Needles left behind after treatment: While every effort is made to collect all needles used during a treatment, occasionally a needle may be left behind either on the pet or in the environment after a treatment concludes. Should this occur, any found needles should be removed promptly and disposed of safely. Needles used in acupuncture are extremely thin and not hollow, so risk of injury is extremely rare.

    Blood: Some forms of acupuncture use hemoacupuncture (where a hypodermic needle is inserted in certain acupoints) to draw and release blood. In other acupoints, a few drops of blood may be seen once an acupuncture needle is removed (this is also normal). If the amount of blood seen is concerning or unexpected, HolisticalVets LLC will recommend further workup at a full service veterinary facility to see if there is an underlying cause that may be contributing to the amount of blood seen. Please inform HolisticalVets LLC if the sight of blood is troublesome. 

    Moxibustion: While this treatment modality is great for adding warmth to treatments when needed and great care is taken not to place the moxibustion stick directly onto a pet, please be aware that there is always a small risk for a pet's haircoat to be singed or for ashes to fall onto skin/haircoat due to the moxibustion stick being in close proximity to the pet. In addition, certain types of moxibustion sticks, while considered more effective than their smokeless counterparts, produce smoke and a corresponding odor similar to that of marijuana. If moxibusion is indicated for your pet's treatment, please inform HolisticalVets LLC if you prefer smokeless moxibustion be used instead. 

    Adverse reactions (e.g., allergic reaction, gastrointestinal upset, etc.): Regardless of how safe something is said to be, just like with humans, there is always a risk of an adverse reaction to food ingredients, herbs, medications, topical products, etc. If a pet experiences an adverse reaction, please stop administering the offending item and inform HolisticalVets LLC. If there is a serious adverse reaction (e.g., vomiting, diarrhea, facial swelling, difficulty breathing, etc.) please contact your primary conventional veterinarian or a veterinary emergency facility immediately. 

    Extremely rare, but possible, complications that can be seen with acupuncture:

    • Stuck needle: Usually easily removed once the affected area is massaged and the pet is walked for a few minutes to loosen muscles
    • Broken needle: Usually easily removed with forceps while the pet is held still
    • Swallowed needle: The needle usually passes on it own through the gastrointestinal system in 5-7 days while the pet is given a high fiber diet (x-rays at a full service veterinary facility are recommended to monitor the location of the needle)
    • Bruising/collection of blood under the skin: Usually resolves with pressure and cold compresses in 24-48 hours

  • Section F: Code of Conduct

    HolisticalVets LLC follows the Golden Rule ("Treat others how you would like to be treated") and applies that rule to interactions with both people and animals. In the same manner, HolisticalVets LLC expects to be treated as such. 

    A valid and legal veterinary-client-patient-relationship (VCPR) begins with a physical exam of the pet and it is formed when there is mutual trust and respect between the client and veterinarian/veterinary entity, as it relies on communication and agreement to grow, foster, and succeed. Without mutual trust, respect, communication, and agreement, it is impossible to help the pet involved, as both parties are unable to work together effectively, which results in a failed VCPR. Without a valid and current VCPR, a veterinarian/veterinary entity is not legally allowed to treat a pet or offer medical advice for that pet. 

    Therefore, it is HolisticalVets LLC's policy to not tolerate any signs of disrespect, rudeness, yelling, screaming, bullying, threats, profanity, name calling, harassment, aggression, demanding or entitled behavior, disparagement, violence, exploitation, unreasonable or excessive demands, and physical, emotional, or verbal abuse, etc., as any of those characteristics is not conducive to a successful VCPR. Any indication of such characteristics will result in an immediate dissolution of the VCPR. HolisticalVets LLC also reserves the right to terminate the VCPR if HolisticalVets LLC's medical recommendations are disregarded in a way that endangers or causes suffering of an animal. Since the VCPR is a two-way street, either the client or HolisticalVets LLC can sever the VCPR at any time and for any reason. 

    Please note: By law, in order to provide any kind of medical advice or prescription for a pet, a physical exam for that pet is required at least once a year to maintain a VCPR. In addition, an updated physical exam is required for any new concerns or a change in medical status.


  • Section G: Additional Information

    If purchasing a product through HolisticalVets LLC, a notice of ten (10) days is required to allow time for the order to be fulfilled. For rush orders that do not provide that 10 day notice, there is an additional $50 processing fee. 


    HolisticalVets LLC does not work with any outside pharmacies; therefore, if an oral sedative is recommended for a future appointment, a written prescription will be given to the client (this allows the client to purchase the sedative from a pharmacy of their choice). If herbs or other products are purchased through HolisticalVets LLC, they will be shipped directly to the client. 


    Medical records generated for the pet by HolisticalVets LLC may be sent to the pet's primary veterinarian to facilitate continuation of patient care. 


    Telemedicine consultation fee: While HolisticalVets LLC is happy to answer a couple of uncomplicated questions that may arise outside of appointments without an additional charge, a telemedicine consultation fee (current rates can be seen here) will be charged for email communication that requires substantial additional time and/or research in order to provide the client with appropriate medical advice and correct information. If HolisticalVets LLC determines that a telemedicine consultation is indicated based on the question(s) received, the client will be sent an email relaying that the question(s) can be answered if the client agrees to the telemedicine consultation charge. If the client agrees and pays the invoice, the telemedicine consultation will be performed and completed within 72 business hours of the invoice being paid (excluding any information from 3rd parties that may take longer). If the invoice is not paid by the due date, the telemedicine consultation will not occur and the question(s) will be discarded without further notice.


    Product consultation fee: There is an additional charge (current rates can be seen here) for questions about diets not formulated by HolisticalVets LLC and non-prescription supplements and medications. This is due to the fact that additional time and research is required in order to provide the client with appropriate medical advice and correct information. If a client is interested in a product consultation and relays the request during an appointment, the fee will be charged to the client's vaulted credit card used for the appointment. If the client relays the request outside of an appointment, the client will be sent a separate invoice prior to the product consultation being performed. If the invoice is paid, the product consultation will be performed and completed within 72 business hours of the invoice being paid (excluding any information from 3rd parties that may take longer). If the invoice is not paid by the due date, the product consultation request will be discarded without further notice.


  • Section H: Financial Consent

    Clients bear full financial responsibility for all charges incurred, and payment(s) for charges incurred will be processed using the credit card submitted during the appointment request. The credit card information is securely stored ("vaulted") with Stripe to facilitate the payment process. All financial transactions are conducted through Squarespace and Stripe, both of which comply with PCI standards to ensure financial safety and security. Clients have the option to request the removal of their credit card information from storage by submitting a written request via email to info@holisticalvets.com after all incurred charges are paid in full.

    Please note that HolisticalVets LLC does not have access to or retain complete credit card details, whether offline or online, and is only able to see the cardholder's name, the associated credit card processor (e.g., Visa, Mastercard, American Express), the last four digits, and the expiration date of the credit card.

    Please be aware that it is a requirement for clients to have a stored/vaulted credit card in order to receive services and products. If an appointment is scheduled or a product/service is requested without a vaulted credit card, the client will be sent a secure link to update their credit card information. HolisticalVets LLC can only proceed with rendering the indicated services and providing the indicated products once the client's credit card has been securely stored/vaulted.

  • Section I: Rescheduling/Cancellations

    Rescheduling

    To reschedule your appointment with HolisticalVets LLC, please send an email to info@holisticalvets.com as soon as possible. We kindly ask that you notify us of any rescheduling requests at least 48 hours prior to your scheduled appointment. 

    Please note that in the event we receive a rescheduling request less than 48 hours prior to the scheduled appointment time, an additional $50 charge will be applied to the credit card on file. This charge covers the costs associated with accommodating last-minute schedule changes and will be charged at the time of rescheduling, prior to the appointment taking place. We appreciate your understanding as HolisticalVets LLC is a mobile business, and our travel routes are planned approximately a week in advance.

    HolisticalVets LLC strives to maintain the utmost commitment to your scheduled appointments, but unforeseen circumstances may arise. In the event that HolisticalVets LLC needs to cancel an appointment prior to the scheduled time due to factors such as unsafe weather conditions or illness, we will make every effort to accommodate your rescheduling needs if possible. You will have the option to select another suitable day and time that works well for all parties involved. We want to assure you that if HolisticalVets LLC cancels an appointment prior to the scheduled time, there will not be an additional $50 fee. Our priority is to ensure a smooth and convenient rescheduling process for you.


    Cancellations

    To cancel your appointment with HolisticalVets LLC, please send an email to info@holisticalvets.com as soon as possible. We kindly ask that you provide a minimum of 48 hours’ notice for any cancellations. Please understand that if you cancel your appointment less than 48 hours prior to the scheduled time or if HolisticalVets LLC is unable to provide services due to circumstances beyond their control (such as unsafe or hostile conditions, aggression, an inaccessible or unavailable patient, breached code of conduct, patient fear, anxiety, or stress, the client not being available for the scheduled appointment, etc.), the credit card on file will be charged all the applicable charges associated with that particular appointment.

    However, if you cancel your appointment between 48 and 120 hours (5 days) prior to the scheduled appointment time, a $200 fee will be charged to the credit card instead of the normally applicable charges associated with an appointment. We would like to emphasize that these cancellation charges are necessary to account for the time, effort, and resources HolisticalVets LLC has dedicated to preparing for your appointment.

    There are no cancellation charges if you cancel your appointment more than 120 hours (5 days) prior to your scheduled appointment time.

    In the event that HolisticalVets LLC determines that it is unable to provide services to you and your pet for any reason prior to your scheduled appointment time, and if rescheduling the appointment is not a viable option, we want to assure you that your supplied credit card will not be charged. Our utmost priority is to ensure transparency and fairness in our service offerings, and we believe it is important not to charge for services that cannot be delivered as planned due to circumstances within our control. 


    Human Health

    While not a requirement, HolisticalVets LLC would greatly appreciate it if all individuals present during appointments wear a face mask correctly, regardless if the appointment is held indoors or outdoors. This is to protect individuals/clients at the appointment, other individuals/clients that are seen later that day/week, and HolisticalVets LLC's staff. If a household member is sick, please email HolisticalVets LLC at info@holisticalvets.com as soon as possible to reschedule the appointment (there is no fee to reschedule an appointment due to illness). 

    HolisticalVets LLC reserves the right to end the appointment and/or VCPR immediately (and any payment for that appointment will be non-refundable and non-transferable) if it is apparent that an individual at the appointment is sick. This is to minimize the risk of spread to other individuals, as HolisticalVets LLC takes the safety and health of all individuals very seriously. 


    Treatment Packages

    Treatment packages have a one-year expiration from the date of purchase and are non-refundable. They cannot be transferred to a different household. In certain cases, and pending approval by HolisticalVets LLC, partially used packages (within one year of purchase) may be transferred to other pets within the same household.

    Please be aware that your credit card will be charged for each appointment as it occurs. For instance, if you purchase a package with four appointments, for each appointment, your supplied credit card will be charged 1/4 of the purchase price of the package (in addition to the travel fee).

    If any appointments remain unused after one year from the date of purchase, the remaining appointments will be charged at the rate specified in the purchased package when that one-year period concludes.

    If HolisticalVets LLC terminates the VCPR before all appointments in a treatment package are used, the supplied credit card will be charged the remainder of the full non-discounted price for the completed appointments. Unused appointments will not incur any charges.


  • Section J: Media Consent

    HolisticalVets LLC would love to take pictures and/or videos, etc. of your pet and post them on its website, social media, brochures, and business cards, etc., but will not do so without your consent.

  • By signing this consent form, I am 18 years or older and the legal pet owner (or authorized agent for the legal pet owner) of the above mentioned animal and have the ability and authority to complete and sign this form. By filling out the required information and signing below, I fully understand the contents of this document in its entirety, agree and consent to them, and hereby voluntarily give my permission and consent for the above mentioned pet to be treated by HolisticalVets LLC. I intend for this consent form to cover the entire course of treatment for the above mentioned pet’s present condition as well as any future conditions for which I seek treatment for from HolisticalVets LLC.
