SSPP K-8 Application Form

SSPP admits students of any race, color, and national and or ethnic origin to all the rights, privileges, programs, and activities accorded to or made available to students at this school. Applications are currently accepted on a rolling basis throughout the year. An application does not guarantee acceptance into the school.

A $100 application fee is required of all applicants and will be collected upon completion of this form.

If you are applying for more than one child, you will need to fill out one form per child and pay the $100 application fee for each application submitted.

Before you begin: you will need to gather some materials to complete this application that may include a family photo, current transcript, immunization forms, etc. Your application cannot be accepted without submitting all of the required materials.
