    • You MUST upload a photo of the front and back of your Credit Card for all Non-Warrantied repairs. If you have a a shipping account, please provide the account number for the return shipping costs. NOTICE: Recalled Products do not require a credit card. All other warranty or repair transactions will not be processed without a credit card. If your PEMF device's repairs are completed under the Standard Warranty terms and you have provided a return shipper account number, the credit card below with NOT be used., otherwise the credit card will be used for Return Shipping Fees.

      Please be advised, an Authorization will appear on your card as follows:

      • $50 - tentative Return Shipping Fees (likely less)
      • $250.00 -$500.00 - tentative Out of Warranty Repair Charges 

      When the transportation and/or repair charges are invoiced, the above authorization will insure the actual credit card charge will not be declined.

      Important: If the charges for out of Warranty Repair is less than $100 the factory will not seek approval nor send a repair estimate.They will simple repair and ship back and charge your card for the repair and shipping fee.

    • See the Tariff code above that you should use.  We typically put the declared and customs value at $1.00 as it’s a repair

      Include the description “NOT SOLD (COMMERCIAL) Warranty Repair Pulse Generator for Animals”,  or if the device is a muman model indicate "Exercise Equipment"

      Regarding potential issues around USA or Canadian Custom Inspections, please include inside the package a note that says:

      • Your name: {name} 
      • The RMA # below  
      • The Device's Serial #: {serialNumber41}
      • Basic Reason for the repair: {input15}
      • Your return shipping address: {addressWhere}
    • Finalizing your Service Request
