First Name*   Last Name*   (“I”), the legal guardian of   First Name*   Last Name*(“Participant” and
“He/His”), understand and acknowledge that as a participant in The Battle Cage Camp, he may be exposed to a potential risk of body and person. I, hereby release AWBC & GOOD Kids Inc. and any of its officers, directors, employees, agents, licensees, subsidiaries, consultants,
independent contractors and affiliates from any and all liability for property damage, grievous bodily harm or
other personal injuries (including those caused by pandemic), death or other claims arising from or in
connection with his participation in The Battle Cage Camp, including claims that are known or unknown, foreseeable or
unforeseeable, future or contingent.

This release also provides AWBC and GOOD Kids Inc. the absolute, irrevocable, and unconditional right
and permission with respect to the photographs/video that they have taken of the Participant, which may be
included with others. AWBC has the ability to use, to re-use, publish and republish in whole or in part,
individually or in conjunction with other photographs, in any medium (including but not limited to print, video or
digital formats) and for any purpose whatsoever, including but not limited to: illustration, promotion, advertising
and trade.

I agree that I will not now or at any time in the future, directly or indirectly, pursue, commence or prosecute any
action, suit or other proceeding against AWBC & GOOD Kids Inc. and any of its officers, directors, employees, agents,
licensees, subsidiaries, consultants, independent contractors and affiliates; Thus, any action, suit or
proceeding arising out of or relating to the actions, causes of action, claims or demands are hereby waived,
released and discharged by me.

I agree, consent and affirm, and, on behalf of Participant minor to be bound by this Injury Waiver and General
Release Form. I represent, warrant and agree that I as Parent/Guardian am/are entitled to the care and
custody of Participant minor and am/are Participant’s legal guardian(s); during his current minority and for a
reasonable time afterwards; that I as Parent(s) hereby acknowledge(s) that I have read the Injury Waiver and
General Release Form and are satisfied that it is fair and equitable for the benefit of the Participant and that I
as Parent/Guardian will not revoke this consent and approval.
