This form should be used to inform FutureQuals of changes to a centre's details including:
- Section 1 - Change of name and/or business ownership.
- Section 2 - Additional site.
- Section 3 - Change of Head of Centre, owner or quality lead, or additional centre officers.
- Section 4 - Additional centre staff (employed staff only)
Be aware that some changes could affect the risk level, centre approval and/or qualification approval status.
Changes to centre name, where the ownership of the centre has changed will be subject to additional due diligence checks. If the Registered Company number has changed a new centre application will be required.
