    Change of Centre Details Form

  • This form should be used to inform FutureQuals of changes to a centre's details including:

    • Section 1 - Change of name and/or business ownership.
    • Section 2 - Additional site.
    • Section 3 - Change of Head of Centre, owner or quality lead, or additional centre officers.
    • Section 4 - Additional centre staff (employed staff only)

    Be aware that some changes could affect the risk level, centre approval and/or qualification approval status.

    Changes to centre name, where the ownership of the centre has changed will be subject to additional due diligence checks. If the Registered Company number has changed a new centre application will be required.

    • To request approval for additional qualification(s), use the Additional Qualification Application Form.
    • To request approval for additional workforce to an existing qualification approval use the Additional Workforce Application Form.
    • To request access to our systems (QMIS, SharePoint, XAMS, Learner Resources) please complete the Systems user access application form.
    • All forms can be found in the existing centres area on the FutureQuals website.

    The Head of Centre must approve all changes. By signing below the Head of Centre confirms that the information provided above is correct and that they approve the change(s).
