FIREARMS TRAINING AGREEMENT - RELEASE AND WAIVER OF LIABILITY

The instructor will provide training and instruction on the required legal topics associated with concealed weapons carry in your state; however WILL NOT provide legal advice or opinion. The instructor is NOT representing any Law Enforcement Agency.

WARNING: THIS DOCUMENT LIMITS YOUR LEGAL RIGHTS. THE USE OF FIREARMS is DANGEROUS

I {name14} hereby assume all risks by agreeing to the following: 1. that I am voluntarily participating in training/instruction by Prime Defense Firearms Training LLC./Shawn Crowl & Jerry Crowl or their legal representatives. I understand the risks and hazards involved, and I recognize serious life-threatening injuries or death can occur while carrying or using a firearm, participating in class, or later from the materials presented, whether caused by the passive or active negligence of Prime Defense Firearms Training LLC. or its representatives or other participants in the class. 2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in class. I represent that I am physically fit, and I have no medical, psychological, or any other condition, which would prevent my full participation in the firearms class. 3.I realize that liability may arise from negligence or carelessness on the part of the persons or entities, being released, from dangerous or defective equipment or property owned, maintained, or controlled by them or because of their possible liability without fault. 4. In consideration of being permitted to participate in the activities of the firearms training class, I agree to assume full responsibility for any and all risks, injuries, or damages, known or unknown, which I might incur or create as a result of participating in the class. 5. In further consideration of being permitted to participate in the firearms training class, I, my legal heirs, executors, administrators, next of kin, successors, or legal representatives knowingly, voluntarily, and expressly waive, release, discharge, hold harmless and promise to indemnify and covenant not to sue Prime Defense Firearms Training LLC., their agents, facilities owner, officers, director, shareholders, employees, instructors, volunteers, or representatives from any and all injuries or damages of whatsoever kind and nature that I may sustain as a result of participating in the firearms training class, or of direct or indirect use of the instruction of the class.I understand that this type of training may result in minor injuries such as bruises and lacerations. 6.1 certify that I am over the age of twenty-one (21) years and I am legally entitled to own and use a handgun and/or firearm. 7. I agree that any provisions of this release are held to be invalid, nevertheless, the balance of the release shall continue in full force and effect. 8. I agree that if the instructor believes that I am or will be involved in criminal activity, the instructor will expel me from the class with no refund. I also agree to hold the parties covered in this waiver are not liable for slander and/or libel as part of being expelled. 9. I agree that if the instructor believes that I am or will be unsafe or dangerous, the instructor will expel me from the class with no refund. I understand that photos and video may be taken during the course and agree that my image may be used for advertising and that extreme close-up frontal facial angles and names will not be published without prior approval. 11. I understand that I must sign this release of liability and fully comprehend the structure and nature of the class, and the risks involved before being allowed to participate in this class. I have read the above release and waiver of liability and full, understand its contents.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Prime Defense Firearms Training LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Prime Defense Firearms Training LLC cannot guarantee that will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other Prime Defense Firearms Training clients and

I voluntarily seek services provided by Prime Defense Firearms Training LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply withall set procedures to reduce the spread while attending my class.

I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of

I have not traveled to a highly impacted area within the United States of America in the last 14 days. I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities. I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19. I hereby release and agree to hold Prime Defense Firearms Training LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of Prime Defense Firearms Training LLC, or that may

otherwise arise in any way in connection with any services received from Prime Defense Firearms Training LLC. I understand that this release discharges Prime Defense Firearms Training LLC from any liability or claim that I, my heirs, or any personal representatives may have against Prime Defense Firearms Training LLC with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Prime Defense Firearms Training LLC. This liability waiver and release extends to the Prime Defense Firearms Training owner, partners, and assistants. I have been informed of all risks involved and I voluntarily agree to the terms and conditions stated above.
