In consideration of Volunteer’s relationship with Charlotte Mecklenburg Dream Center/Restoring Place Church
as a volunteer, Charlotte Mecklenburg Dream Center/Restoring Place Church’s acceptance of Volunteer as a volunteer, the mutual promises and covenants hereinafter set forth, and other good and valuable consideration, the receipt and sufficiency of which the parties hereto acknowledge, the parties agree as
follows:

1. Acknowledgments. Volunteer acknowledges that in his/her capacity as a volunteer for Charlotte Mecklenburg Dream Center/Restoring Place Church, he/she may have the opportunity to become aware of confidential information concerning the financial gifts of members and staff, operational activities of Charlotte Mecklenburg Dream Center, and/or other information of a private, sensitive, or privileged nature.Volunteer further acknowledges that he/she is aware that all such information must be held in the strictest confidence and is vital to the continuing success of Charlotte Mecklenburg Dream Center/Restoring Place Church, and that dissemination of it could result in significant damage to Charlotte Mecklenburg Dream Center/Restoring Place
Church.

2. Venue. This agreement applies to all Volunteer’s activities as a volunteer associated with Charlotte Mecklenburg Dream Center/Restoring Place Church, wherever located.

3. Survival. To the fullest extent permitted by law, Volunteer’s obligations to ensure and protect the confidentiality of the Confidential Information set forth in this Agreement shall survive the expiration, rescission, or termination, as the case may be, of this agreement and shall survive the termination of the relationship between Volunteer and Charlotte Mecklenburg Dream Center/Restoring Place Church and the
termination of Volunteer’s service to Charlotte Mecklenburg Dream Center.

4. Applicable Law. This agreement will be governed by the laws of the State of North Carolina, without regard to its conflicts of laws principles. Volunteer and Charlotte Mecklenburg Dream Center/Restoring Place Church expressly consent to the personal jurisdiction of the State and Federal courts located in the State of North Carolina for any lawsuit filed there arising from Volunteer’s volunteer activities.

5. Status of Volunteer. This agreement does not create an employment relationship between Volunteer and Charlotte Mecklenburg Dream Center. Volunteer acknowledges that this agreement is a volunteer agreement,
not an employment agreement.

The information contained in this application is correct to the best of my knowledge.
In consideration of the receipt and evaluation of the application by Charlotte Mecklenburg Dream Center, I release any individual, church, organization, employer, reference or any other person or organization,both collectively and individually, from any and all liability for damages of whatever kind or nature which may at any time result to me, my heirs, or family on account of compliance or any attempts to
comply with this authorization. I waive my rights that I may have to inspect any information provided about me by any person or organization identified by me in this application.

Should my application be accepted, I agree to be bound by expectations, polices and guidelines of Charlotte Mecklenburg Dream Center, the volunteer expectations and to maintain Christ-like conduct in the performance of my services on behalf of Charlotte Mecklenburg Dream Center.

I further state that I HAVE READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND I SIGN THIS RELEASE ON MY OWN FREE ACT. This is a binding agreement that I have read and understand.

In consideration of the Charlotte-Mecklenburg Dream Center (CMDC), Restoring Place Church (RPC), & The King’s Kitchen (KK) allowing me to participate with helping feed, minister to and provide goods to those living on the streets, the undersigned volunteer hereby agrees to donate and offer their personal services, time and labor free of charge to CMDC, RPC & KK, all of which are not-for-profit 501c3 organizations. Volunteer understands and agrees that they are not an employee of CMDC, RPC & KK and are not entitled to any wages and/or benefits associated with their services. Volunteer acknowledges and understands that the activities include work that may be hazardous to the volunteer.

Volunteer further agrees, to release and forever discharge CMDC, RPC & KK, its owners, directors, officers, employees, agents, volunteers, invitees, sponsors, cash donors, in-kind donors and all other persons or entities whomsoever, of and from any and all actions, claims, and demands, known or unknown, which Volunteer has or may have in the future arising out of volunteering for CMDC, RPC & KK.

Volunteer understands that this Release discharges CMDC, RPC & KK for any liability or claim that the Volunteer may have against CMDC, RPC & KK with respect to any bodily injury, personal injury, illness, death or property damage that may result from Volunteer’s activities with CMDC, RPC & KK whether caused by the negligence of CMDC, RPC & KK or its owners, directors, officers, employees, agents, volunteers, or otherwise. Volunteer understands that CMDC, RPC & KK does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of any accident, injury or illness.

Volunteer is aware that photos/videos taken during events may be used in future social media and marketing materials and consents to their image being included in these photos/videos.

 *No one under the age of 18 is allowed to participate in Friday night street ministry
