Terms and Conditions

I hereby enter into this agreement in consideration of the gift provided by the West Slope Foundation (WSF).IMPORTANT NOTICE: BY SIGNING THIS AGREEMENT YOU ARE GIVING UP CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO RECOVER DAMAGES IN CASE OF INJURY, DEATH, OR PROPERTY DAMAGE, ARISING OUT OF YOUR USE OR MISUSE OF SAID GIFT.READ THIS AGREEMENT CAREFULLY BEFORE SIGNING IT. YOUR SIGNATURE INDICATES YOUR UNDERSTANDING OF AND AGREEMENT TO ITS TERMS. By signing this form, I hereby acknowledge on behalf of myself and anyone living within my household will not attemp to sell, return, exchange, or attemp to receive the cash value for any item(s) recieved. If WSF finds that you or anyone within your household breaks this term, you and everyone in your household will be terminated from participating in any of WSF programs.

By signing this I hereby acknowledge that although there may assumed risks by accepting gifts. I agree to indemnify, save and hold harmless WSF and its principals and agents from and against any loss, liability, damage, attorneys’ fees, or costs that they may incur arising out of or in any way connected with either my acceptance use or misuse of the WSF’s gift. By signing this Agreement, I hereby acknowledge my complete understanding, agreement and consent without restriction, without liability to, its principals or agents, and with full knowledge and understanding of the disclosures, waivers, and releases herein with the use and operation of all items received.
