  • The University of Colorado Denver welcomes you as a participant in this activity, including the use of University of Colorado Denver facilities and equipment. Please read through the following important information.

    The University of Colorado Denver does not provide health insurance for individuals participating in activities made available or sponsored by the University of Colorado Denver. As such, you or your personal health insurance will be responsible for payment of medical services and care for any injuries sustained during the designated student organization activity. By accepting the terms of this form, I confirm that my personal health/medical plan covers student organization activities.

    I exercise my own free and voluntary choice to participate in the designated activity, including use of facilities and equipment provided by the University of Colorado Denver. I understand and assume all associated risks of the designated activity. These risks include but are not limited to those listed below.

    Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, thereis still a risk of serious injury when participating in any activity/program. Understandably, not all hazards and dangers can be foreseen. Participants must understand that depending upon the particular activity, certain risks, dangers and injuries due to natural occurrences beyond human control or influence can occur. These may include: community spread disease, being in an urban environment on and off the campus; participating in activities in a high altitude environment with/without acclimation; inclement weather; tick and insect bites and illnesses from the bites; head, back and limb injuries from slip and falls. Pre-existing medical conditions can be worsened and can trigger; allergies, asthma, diabetes, heart disease, neurological illnesses, vertigo, dehydration, heat exhaustion or altitude sickness. You may also encounter; scrapes, bruises, skin rashes, sunburn, and adverse reactions to dangerous/irritant plants or chemicals. In addition, inadequate or defective equipment, inadequate supervision or instruction, carelessness, horseplay, vehicle accidents and all other circumstances inherent to these activities/programs may exist and contribute to an injury.

    I agree to assume all risk of personal injury or loss, bodily injury (including death), damage to or loss or destructions of any personal property occurring in connection with or arising out of participation whichis inherent to student organization activity. In consideration of my acceptance as a participant in this student organization activity, I hereby release and discharge, indemnify and hold harmless the Regents of the University of Colorado, a body corporate and any other persons or entities acting on their behalf, and their successors and assigns for any and all claims, demands, costs and expenses, and causes of action whatsoever, either in law or equity, arising out of or in any way connected with any loss and/or bodily injury and/or disability, arising from my participation in the student organization.

  • Participants in university activities are sometimes photographed and videotaped for use in University of Colorado promotional, educational and research programs. Such audio, video, film and/or print images may be edited, duplicated, distributed, reproduced, broadcast, and/or reformatted any form and manner without payment of fees.

    Inthe event of an emergency, I grant the University of Colorado permission to authorize emergency medical treatment for the duration of participation in the student organization activity. I have had sufficient time to review and seek explanation of the provisions contained above, have carefully read them, understand them fully, and agree to be bound by them. After careful deliberation, I voluntarily give my consent and agree to this Release, Assumption of Risk and Waiver.

    Ihereby certify that I have read and understand the provisions above and agree to them. For participants under 18 years of age, the parent or guardian accepts the above terms and grants permissions for the student's participation on behalf of said minor, as permitted by C.R.S. $ 13-22-107.
