  • Part 3: Qualification Workforce

    In this section you are required to provide details of the individuals that will be involved in the delivery, assessment and/or IQA of the qualification and attach evidence (if not already available on your centre SharePoint site) to show how each individual meets the pre-requisite requirements for that role. This should include CV, CPD records, certificates and license details for driving qualifications. Failure to provide the required evidence may result in your application being rejected. You can include up to 5 individuals in this section; if you have additional workforce individuals that you need to register for this qualification, please use the additional workforce application form. All workforce individuals must be approved by FutureQuals before they are deployed by you in the delivery of this qualification and their details will be included on our facilitator register. You must list at least 3 individuals (covering at least 1 Tutor, 1 Assessor, 1 IQA and where more than 1 IQA, who will be the 1 Lead IQA). Depending on the qualification selected, there may be additional requests for evidence. Workforce individuals details will be added to our facilitator register

  • Part 4: Centre Declaration

  • By clicking submit I am confirming the following statements

    • I confirm that the Centre will meet all the requirements of FutureQuals Approval Criteria as detailed on the FutureQuals website in respect of this application and that the details are, to the best of my knowledge, correct.
    • I confirm that all staff members involved in the delivery of the qualifications are occupationally competent.
    • I confirm that the Centre will collect all personal data in accordance with the Data Protection Legislation, and in particular that it has the consent of the learner.
    • I accept that FutureQuals will hold and process electronically the information given and may use it for any purpose deemed relevant to the product.
    • I confirm that the Centre will work with FutureQuals to protect learners’ interests if centre or qualification approval ends.
    • I confirm that the Centre’s Senior Management Team supports this application to gain additional qualification approval.
    • I confirm that the Centre has the appropriate infrastructure, equipment and facilities to securely deliver assessments in both electronic and paper-based formats where appropriate.
    • I confirm that the Centre will comply with FutureQuals policies and procedures for invigilation of assessments and security of examination materials.
