Lasting Power of Attorney (LPA)

A legal binding document made by the Donor (the person creating the LPA) who appoints a Donee to make decisions on his/her behalf and is effective when s/he lacks the mental capacity.

2. A $50 surcharge applies for any scheduled doctor and nurse visit on a Public Holiday.
3. A $30 surcharge applies for any scheduled doctor visits to Sentosa, Jurong Island and Tuas.
4. Rates are not applicable with other on-going promotions, discounts, offers, vouchers, rebates, privileges, or corporate and other programmes.
