    Please follow the instructions below to apply for a Lakewood UEZ loan.
  • The Lakewood UEZ limits a business to a maximum of $100,000 in loan dollars per business. A business may have only one loan type at a time, i.e. one Microloan, not two Microloans. A business may have up to two loan types concurently subject to Finance Committee review.

    Applicants who have not completed their requirements on an application within 90 days from the submission date will have their application purged from the system.

    Applicants must maintain continuous Active UEZ Registration status for a minimum of at least Three (3) years. If Applicant does not maintain active status for a minimum of Three (3) years, the Applicant forfeits its ability to apply for any UEZ loan programs for a minimum of Five (5) years from date of UEZ Registration Inactive status.

    Handwritten forms will be rejected.

    1. Prepare a business plan. You can download a sample business plan by clicking here.
    2. Download the following PDF files
      • Application Document Checklist
      • Loan Application
      • Business Plan Template
      • Cash Flow Projections
      • Balance Sheet
      • Profit & Loss
    3. Generate a UEZ Tax Clearance Certificate
      • Make sure the 'Agency:' section on the Certificate states 'New Jersey Department of Community Affairs'.
      • If you are not able to obtain a Tax Clearance Certificate, please contact UEZTax.Clearances@treas.nj.gov to fix your tax compliance issue then come back and try again.
    4. Print out and upload your 'Notice of Certification Application Approved' email you received from UEZdonotreply@dca.nj.gov as proof of registration in the program.
    5. Fill out all the PDFs completely.
    6. Retreive a copy of your business federal tax return for the last two years.
    7. Retreive a copy of the personal federal tax return for all principals for the last two years.
    8. Retreive a copy of your Certificate of Formation or Incorporation.
    9. Print out and upload a copy of your Business Registration Certificate (BRC).
    10. Use the form below to upload the PDFs and submit the application.

  • Personal Financial Statement for all principals of the business

    (Please disclose any family financial assistance you may receive and provide a PFS for anyone who may be guaranteeing the loan other than the principals.)
