CRIMINAL RECORD & VULNERABLE SECTOR CHECK
The following individual is a volunteer for the Fort Saskatchewan Minor Hockey and has consented to a Criminal Record Check (CRC). The FSMH acknowledges that volunteer positions vary in the scope of duties and interaction with our participants and that a Vulnerable Sector Check (VSC) is not necessary for all volunteers.

Some of these volunteer positions require a VSC;( Coach, Trainer, Assistant Coach). as their duties may include direct physical contact with our participants. These volunteers at times will be in situations that they might be alone with participants in cases such as taking players to the washroom, helping players with donning and doffing equipment, providing medical attention, providing transportation to and from games and practices. etc.

Some of these volunteer positions require a CRC, as their duties include finances and/or direct access to the personal information of our participants. (Manager, Treasurer, and on ice help)

FSMH understands that it is possible that certain volunteer roles within our organization, may place this volunteer "in a Position of Power", as our participants are 4 to 17 years of age. FSMH has Policies and Procedures in place to mitigate risk to the participants in our programs, and to the volunteers themselves.

The volunteer will provide the certificate generated to the Fort Saskatchewan Minor Hockey Association.
