All US Foods credit and pickup requests must be e-mailed to US Foods Customer Service and your Sales Representative. Attach this completed form and the REQUESTED PHOTOS below when submitting any credit / pickup request.
- Credits will NOT be processed if any information is missing or incorrect.
- Product being returned due to ordering or shipping errors must be returned un-opened and in perfect, re-sellable
condition. DO NOT WRITE ON THE BOX OR PRODUCT.
- Product being returned for damage or quality reasons must be kept until US Foods picks it up. Product should not be
discarded without approval from US Foods.
