The goal with this form is to get to know your hair goals, which will determine if the service you've selected is the correct one or if we need to make adjustments. Completing this form will help me to be able to create a plan for your first visit and help answer some of your questions along the way. If you are still unsure after completing this form, if I am the right fit for you, I encourage you to schedule a complimentary in salon consultation. Do you have more hair than the average person? I ask this only to make sure we have enough time to give you the service you deserve. My service menu time blocks are set up for the average amount of time it would take me. If you have hair that will take longer, you know this, and it is important that I know it too. I never want you to feel you are being rushed during your service. Please select the appointment with the additional time added when necessary.
