The goal with this form is to get to know your hair goals, which will determine if the service you've selected is the correct one or if we need to make adjustments. Completing this form will help me to be able to create a plan for your first visit and help answer some of your questions along the way. If you are still unsure after completing this form, if I am the right fit for you, I encourage you to schedule a complimentary in salon consultation. Upon submission of this form, you will be automatically directed to my online booking site where you can request an appointment. If you don't find an appointment available right away, please add yourself to my waitlist. When an opening becomes available in the times you selected, you will automatically be notified. My regular business hours behind the chair are on Tuesday, Thursday, and Friday between 9:30 a.m. and 5:30 p.m. Occasionally, I am able to offer appointments outside of these hours at a premium rate, would that interest you?

Do you have more hair than the average person? I ask this only to make sure we have enough time to give you the service you deserve. My service menu time blocks are set up for the average amount of time it would take me. If you have hair that will take longer, you know this, and it is important that I know it too. I never want you to feel you are being rushed during your service. Please select the appointment with the additional time added when necessary.

Cancellation Policy: I understand that if I need to cancel or reschedule my appointment, I need to do so with a minimum of 48 hours notice. If I do not cancel my appointment within the requested time frame, I understand a rebooking fee may be invoiced and may be required to be paid before I book another appointment.

Children in Salon Policy: My studio space is intimate and therefore, for safety reasons and insurance purposes, no child may accompany you while you receive a service. Your understanding is greatly appreciated.

Retail Refund Policy: I understand that I may exchange any opened or unopened retail product within 14 days of purchase. My stylist will suggest an alternative product to meet my needs and the value of the product I am returning will be applied towards the new product. I understand that cash or credit back for returned retail products isn't an option. I understand that retail returns can't be used as a service credit.
