  • Adam Radlaff has been discharged and advised to seek a higher level of care on this 15th day of May 2019. Resident was discharged for the following reason(s):

  • Rent past due in the amount of

  • Breach of Behavioural/Financial Contract.

  • Failure to comply to Serving Institute Rules & Regulations.

  • Failure to Comply with Drug Testing Policy.

    • Resident has come back to the residence after curfew on more than one occasion and under the influence of alcohol/drugs. He refused to take a breathalyzer.

    • Resident refused to sign Behavioral Contract.

    • Resident has difficulty understanding and following rules.

    • Resident was counseled on more than one occasion and shows no progress in his transition or way of living.

    • Resident won't listen to, or follow through on the suggestions he’s given. His attitude is not in line with what Serving Institute Residential Center residents are trying to follow as a program of improvement.

    • Resident understands that he is to vacate the premises within 48 hours in accordance with Florida Statute 397.487.
