REGISTRATION:
Kindly submit this form BEFORE 03 April, we need at least 30 people to attend in order to cover the costs.
One form can be used for more than one family member, include both names in the "First Name" field using "&".
If the attendee is younger than 18 years, the parent / guardian needs to complete the form.

SCHEDULE:
Students may arrive / leave at any time. Parents can call after 20:00.

COST: R800pp. Includes transport to camp, meals, accommodation and activities. 

BRING:
Blanket, Sleeping bag and pillow.
Warm clothing.
Costume and Sun Block.
Toiletries and Towel.
Cutlery, Crockery, dish cloth.
Water Bottle.
Any Medication (specify it on the form below).
Running Shoes.

ALL items are to be CLEARLY marked.
No need for karate suit or belt.
No electronics or money.
No bags of sweets/drink

DIRECTIONS:
Take the N1 to Zambezi Toll Gate (Sefako Makgatho Drive exit).
Take the R513 ( Sefako Makgatho (Zambezi) Road extension) east towards Cullinan for 1 km. At first robot, by the ‘Tshwane China Mall’, turn left onto the R573 towards Moloto, KwaMahlanga for 25km.
Pass Roodeplaat dam, the Pienaars River, the Police Dog School, the Kameelfontein turn-off to right and Walmannsthall turn-off to the left. Cross over Krokodilspruit, rise over next hill, pass under Eskom high tension lines and turn right onto dirt road towards Cullinan. Signposted ‘Boekenhout sand’, ‘ Octagon’ and ‘Bergsig Nursery’ and ‘Paradeisos’ After about 3.2 km look out for a turnoff to the right marked G34, Kamp Reynard, Breytenbach, Hamman.

Please note that Camp Reynard and Goodland Fountain (the main camp) are not located on the same farm, therefore don't just try to use google maps and enter Goodland Estate, because you then will end up at the main complex and not at Camp Reynard..

INDEMNITY:
I, the undersigned, hereby submit my application for myself / my child’s (children’s) participation in the Eagles Karate Camp. I fully understand and accept that any tours, excursions and participation during this period shall be taken at my child's own risk. I hereby on behalf of myself, my executors, my spouse and my child, aforesaid indemnity exempt, hold harmless and absolve the Eagles Sport CC, its owner, employees, staff, officials and parents acting on its behalf against and from any claims whatsoever that may arise in connection with any injury, loss or damage to my child in person or the property of my child, in the course of such tours, excursions, activities or in the course of transportation and conveyance.
