Supplier Declaration

For the purposes of the Louisiana Pacific Building Solutions (Louisiana Pacific) SFI Chain of Custody certification and associated Due Diligence System (DDS), the following Supplier Declaration is provided.

To the best of our company’s (the supplier) knowledge, the fiber/ raw material provided to Louisiana Pacific originates from the location(s) listed below.  Additionally, the species and products supplied to Louisiana Pacific are as indicated.

As it relates to our company's (the supplier) certification status, the company (the supplier) is certified to the following standards:

Please provide a list of your Qualified Logging Professionals (by crew, if appropriate)

The fibre supplied to Louisiana Pacific originates from the location(s) noted below, and may consist of the following species and products:

Indicate the product type(s) that are being supplied to Louisiana Pacific

Indicate the species that are being supplied to Louisiana Pacific

Indicate the source area the fibre was originally harvested

Supplier acknowledges and declares that:

a) to the best of the supplier’s knowledge, the supplied material was sourced legally or harvested with authorization from the appropriate government agency (where required).

b) as requested by Louisiana Pacific, the supplier will make all reasonable attempts to provide information to identify the forest management unit(s) relating to the origin of the raw material (Forest Practice Permit number, Timber Brand/Mark, Forest Tenure number) and the whole supply chain of the supplied material.  It is acknowledged this will NOT include confidential financial information.

c) for the purposes of the chain of custody certification, should an on-site inspection be required to confirm the fibre sourcing (i.e., geographic origin, legality of harvest, and/or supply chain), the supplier/ seller will provide reasonable assistance to facilitate the inspection by Louisiana Pacific or by a relevant third-party certification organization.

d) should information from the supplier change, (i.e., new source area, species, etc.), the supplier will notify Louisiana Pacific and provide an updated Supplier Declaration.
