I understand that I may appear in photographs or videos used for promotional purposes of future boot camp events on the traintoreign.com website. I hereby release all those involved in leading, organizing, and/or hosting this event from responsibility and liability for any illness or injury that I may sustain during any activity, and any and all claims and liabilities. In the event of an emergency, I hereby authorize an adult leader of the activity, as agent for me, to consent to any Xray examination, medical, dental, or surgical diagnosis, anesthesia, treatment, and hospital care advised and by a licensed physician, surgeon, or dentist (as appropriate) licensed to practice under the laws of the state where the services are to be rendered, either at the physician's office or in a hospital. To the best of my ability, if I have been made aware of exposure to the coronavirus within 14 days of the camp start date and/or I am experiencing symptoms related to COVID-19 such as but not limited to: fever, chills, sore throat, headache, difficulty breathing, etc.… I will contact the camp registration team to withdraw from camp. Release, Waiver and Assumption of Risk
This is a legally binding release, waiver, and assumption of risk made by me, to Train To Reign Ministries (individually, "RKM" DBA Train To Reign) and Eagles Wings Lodge (hereinafter, RHM and Eagles Wings Lodge may jointly be referred to as the "Organizations").Train To Reign WAIVER I wish for myself to be able to utilize the facilities located at Eagle Wings Lodge, as well as any off-site facilities where a particular activity is conducted (the "Facilities") during the retreat sponsored by RKM commencing on Thursday, September 21, 2023 and ending on Sunday, September 24, 2024, as well as to participate in any activities sponsored, organized or recommended by the Organizations during such period (the "Activities"), and I hereby agree as follows:I acknowledge and understand that there are certain dangers and risks to which I may be exposed by utilizing the Facilities or participating in the Activities, including the risk of serious physical injury, temporary or permanent disabilit, and death, as well as severe social and economical loss. The risks may arise not only from my own actions, inactions, or negligence, but also from the actions, inactions, or negligence of others, or the condition of the Facilities or of any equipment used. I also acknowledge and understand that there may be other risks not presently known or reasonably foreseeable. I understand that that I am not required to participate in any Activities or to utilize the Facilities, but that I freely choose to do so, despite the possible dangers and risks and despite this release, waiver, and assumption of risk. I hereby assume all risks from my participation in the Activities and utilization of the Facilities.In partial consideration (in addition to monies paid to the Organizations for the opportunity to participate in the Activities and utilize the Facilities of the opportunity to participate in the Activities and to utilize the Facilities, and/or to receive assistance, training, guidance and/or instruction from the personnel of the Organizations, I agree, on behalf of my family, heirs, and personal representative (s), to assume all risks and responsibilities surrounding my participation in the Activities and utilization of the Facilities. To the maximum extent permitted by law, I release and indemnify the Organizations, and their respective trustees, officers, employees, and agents, from and against any and all liability, actions, debts, claims, and demands of every kind whatsoever, specifically including any claim for negligence or negligent acts or omissions and any present or future claim, loss, or liability or injury to person or property that I may suffer, or for which I may be liable to any other person, that may or does arise out of my participation in the Activities and my utilization of the Facilities.I acknowledge that prior to signing this release, waiver, and assumption of risk, I have had an adequate opportunity to read and understand it, and any questions I have had have been answered to my satisfaction.By signing this form I agree to use my best judgment in participating in the Activities and utilizing the Facilities, and to faithfully adhere to all safety instructions and recommendations, whether oral or written. I hereby certify that I am a competent adult assuming these risks of my own free will, being under no compulsion or duress.In the event of any serious physical injury, temporary or permanent disability, or death, I hereby grant permission to the Organizations to contact one or more of the individuals listed on the attached Personal and Emergency Information sheet. I (the undersigned) hereby grant to Train To Reign Ministries, a Ohio non-profit corporation, an individual (hereinafter collectively referred to as "You"), the right to photograph me and to record my voice, performances, poses, actions, plays, and appearances on film, digital media or otherwise, and to use my picture, photograph, silhouette and other reproductions of my physical likeness in connection with the above-referenced Production. I agree that You, in your sole discretion, may edit and otherwise alter the Production and that the Production may be used in whole or in part for any and all broadcasting, webcasting, audio-video products for distribution, promotional and/or exhibition purposes in any manner or media, in perpetuity, throughout the world.I further grant the right to reproduce in any manner whatsoever any recordings including all instrumental, musical, or other sound effects produced by me, in connection with the Production.I understand that there will be no financial or other remuneration for Your authorized use of my physical likeness and sound in the Production as described above. Additionally, I agree that I will not assert or maintain against You, and/or Your successors, assigns and licensees, any claim, action, suit, or demand of any kind or nature whatsoever, including but not limited to those based upon theories of invasion of privacy, rights of publicity, or other civil rights, or for any reason in connection with Your authorized use of my physical likeness and sound in the Production as herein provided.I represent that I have the right to grant to You those rights described in this Release Form, and that my performance and the rights I have granted herein will not conflict with or violate any commitment or understanding I have with any other person or entity.

Cancellation Policies
Cancellation by Registrant: Full payment is required at the time your reservation is made for this event. You may cancel your registration and receive a refund of your registration fee up to 3 weeks prior to the date of the event; provided, however, you will be charged a $75 handling fee upon the cancellation of your registration. In the event that you cancel your registration less than 3 weeks prior to the date of the event, you will forfeit the entire registration fee. Regardless of when you cancel your registration, your registration is not transferable to another event, date, or person.Cancellation by Train To Reign If the event is made impossible by any act of God, war, earthquake, fire, strike, sickness, accident, pandemic, epidemic, act of government, its agencies or officers, or any other legitimate cause beyond our control, we will cancel the event and refund your full Wild at Heart event registration fee to you.
