Spay/Neuter Appointment Request Form

All extra services are add-on's to spay/neuter surgery. They are not offered through Ace of Spays for pets not needing surgery. Should you need assistance with this form or have a question please call us at 609-267-2111.

A $40 deposit is required at the time of scheduling.

Our veterinarian does a brief pre-surgical examination on pets the morning of surgery. If you would like a more comprehensive examination with or without bloodwork, it can be done through our sister organization (same building, similar staff). We will have access to those records. Please sign up for that exam first by filling out this form. After you get your appointment for an exam, come back here to schedule a spay/neuter appointment at least 3 days after the exam.
