Marucci Clubhouse, LLC ("We") would like to take photographs / use photographs / make a video recording of your child / children for promotional purposes. These images may appear in our printed publications, in the media, on video, on our website, our Facebook page, our Twitter page, our Instagram page, or other electronic media. In recognition of you and your child's / children's privacy rights, we need your permission before we use any images of your child / children. Conditions of Use

1. This form must be signed by a parent or guarding for the use of pictures and/or videos for all children under the age of 18.

2. This form is valid for pictures taken one year from the date of signing. The consent you provide will automatically expire after this time.

3. We will not include full names (which means first name and surname) or other personal identification information of any child or adult in an image on video, on our website, or in printed publications, without first obtaining consent.

4. We will not include personal e-mail or postal addresses, or telephone or fax numbers on video, on our website, in printed publications, or in any other promotional materials.

5. We may use group or team images with general or collective labels without including the names of individuals associated with such group or team.
