INFORMATION AND DOCUMENTS TO BE UPLOADED FOR OBTAINING ESTABLISHMENT ENTERTAINMENT LICENCE & AMUSEMENT LICENCE AS PER GO MS NO.129, HOME(POLICE-C) DEPT., DT:02-07-2005 AND OTHER RELAVANT GOVT. ORDERS.

N.O.C. from Asst. Medical Health Officer, G.H.M.C. Hyderabad.
2B Bar Licence issued by the concerned District Prohibition and Excise Officer in case of Bar & Restaurant.
No Objection Certificate from Telangana State Disaster Response and Fire services Department as per requirement / D.F.O, wherever applicable.

Blue print of the site duly showing parking place and built-up area with individual rooms/halls etc. with measurements separately in Sq. Mtrs. approved by the competent authority in GHMC. Parking place to be provided in accordance with the regulation under F.A.R. (Floor Area Ratio Regulations & Standards of building requirements in A.P.) under parking head as laid down in G.O.Ms.No.168, Municipal Administration Department dt.07-04-2012 or as per the latest prescribed Govt. orders and G.O.Ms.No.187 of MAUD (M1) Department Dt:07-07-2017.
Declaration on a non judicial stamp paper of Rs.10/- stating that the designated parking area in Sq. Mtrs. with address of the premises shown in the site plan to be used only for parking purpose and not for any other purposes. Further, wherever there is music, a declaration shall also be included that they will follow the directions including timings given by the Hon'ble Supreme Court of India in WP.No.72 /1998 and other relevant Acts and Judgements issued time to time, ensure that there is no violation an account of noise pollution.
Title deed/lease deed or rental deed, showing the purpose for which the site is leased out, both for the establishment and also for parking space provided in the premises should be specified in the deed with specific measurements.
Partnership deed if there are more than one partner, enabling any one of them to take all licences of various departments in his/her name to run the establishment.
The license is valid for one year.

NOTE:- 1.If any licensed establishment wants to play music then they have to obtain a separate amusement licence from the Commissioner of Police, only after obtaining the Police licence for the Public Place of Entertainment.

2.In case of cancellation of Police licence for Public place of Entertainment to an establishment, the Amusement licence issued to that establishment also stands cancelled automatically.

3.The licenced establishments shall follow all the prevailing Acts & Rules of the relevant period, Government Circulars, Court Judgements and notifications issued by the Commissioner of Police, Hyderabad City, time to time failing which the licencing authority is empowered to cancel the licence without any prior notice.

UNDERTAKING

I hereby declare that all the above information submitted in the application form is correct to the best of my knowledge and that I agreed and aware of that furnishing of incorrect information and any violation relating to Any Law, Rule, Judgement or Notification of the licencing authority of the relevant time, the licence will be cancelled by the Commissioner of Police, Hyderabad City without any prior notice.
