I hereby agree to accept and abide by the terms of this Membership Application and Agreement. I understand that this membership agreement is for a term of twelve (12) months, 6 months or month to month as specified above and will continue thereafter on a month-to-month basis unless cancelled by me within thirty (30) days advanced written notice. I understand that if I do choose to cancel prior to the twelve-month contract commitment, I will pay 50% of the months remaining in my contract in full at the time of my cancellation. This amount will be electronically transferred from the account used to pay monthly dues unless authorized by Rise Owner, Jenny Roe. (EX. Contract signed in January, you cancel in April. You will pay 4 months out of 8 remaining months in full). In the instance of an Emergent situation, to be discussed between Jenny Roe (owner) and member, the terms of this contract are subject to change.

I hereby authorize Rise Womens Fitness & Wellness to effect payment for agreed upon monthly dues and approved membership charges for the duration of my membership through electronic funds transfer. This authorization is to remain in full effect until Rise Womens Fitness & Wellness has received WRITTEN NOTIFICATION from me 30 days prior to cancellation date. I understand that if I do choose to cancel prior to the twelve month contract commitment, I will pay 50% of the months remaining in my contract in full. (EX. Contract signed in January, you cancel in April. You will pay 4 months/8 remaining months in full)

I understand that it is my responsibility to have sufficient funds available in my account on the transfer date in order for Rise Womens Fitness & Wellness to make the automatic transfer. I acknowledge if sufficient funds are not available to cover the amount of my transfer, I will be charged a return fee of $50.00. I also acknowledge that if I fail to pay my membership fee for two consecutive months, I will be discharged as a member of Rise Womens Fitness & Wellness. I understand that at this time Rise Womens Fitness & Wellness will also effect payment in full for all months remaining in my contract. I further acknowledge that Rise Womens Fitness & Wellness will not be liable for any charges including but not limited to: any charges related to items returned due to insufficient funds.

I, name agree to release and hold harmless Rise Womens Fitness & Wellness, and its employees from any loss, liability, claim of bodily injury or property damage or costs which may arise due to my use of Rise Womens Fitness & Wellness's equipment and my participation in Rise Womens Fitness & Wellness's programs.
