Adopting a Family Pet is a long-term commitment. We want our applicants to be prepared for that commitment and dedicated to a high level of care for their new companion pet. To ensure this, Raintree Jacks has the following adoption process:

Step 1. Fill out an Adoption Application below. If you are having trouble submitting the form, email raintreejacksrescue@gmail.com for an application and alternate instructions on submission. Once you have completed the application, DON'T forget to click the SUBMIT button. Incomplete Applications WILL NOT BE PROCESSED.

Step 2. Raintree Jacks will review your application and verify the information provided. This includes property/landlord verification and reference checks. This can take a week or longer depending on how quickly we receive responses.

Step 3. We will contact you regarding your application, along with a telephone/in person interview. This allows us to get to know you, and to answer questions you may have. We do not set an appointment with you for a home visit but we do check on your property to insure it is a suitable environment for the dog you have chosen.

Step 4. We will complete a review of all findings and contact you with a final decision. Upon approval, an appointment can be set for a meet and greet.

We are dedicated to saving as many lives as we can and understand that the faster this process is completed, the faster we can take another dog into safety. Please be sure to provide us complete, detailed information and all contact information is correct to minimize delays.

Introduction

Thank you for your interest in adopting a dog from Raintree Jacks Inc. Whether you adopt from us, or another rescue or shelter, we commend your decision to adopt and not buy a pet store puppy.

Please do not fill out this application if you are not serious about adopting from Raintree Jacks Inc.

We only place dogs in homes located in the United States.

The application is the first step our adoption process. An approved application is required prior to making an appointment to meet a dog.

The base adoption fee for dogs over 6 months of age is $350. Puppies are $375. This fee also includes spay or neuter (unless under 6 months of age), vet exam, rabies, DAPP, and Bordetella vaccines and heartworm testing. The adoption fee must be paid in full via Paypal, Venmo or cash at the time of adoption to secure a particular dog. NO PERSONAL CHECKS will be accepted. This adoption fee is NOT refundable.

If puppies are under 6 months and too young to be altered, ADOPTER will sign an addendum to this adoption contract stating they will have the puppy spayed or neutered at their expense before the puppy reaches 8 months of age.

Acceptance Criteria

Upon execution of this contract by both parties, the adopted dog will become the sole responsibility and property of the ADOPTER. From that time on, ADOPTER, agrees to care for, be responsible for, and give due diligence to the needs of the adopted dog.

The ADOPTER, understand and agree to the following specific stipulations:

1. Provide good quality food and always keep clean water available.

2. Patience and understanding will be exercised with the dog.

3. All veterinary needs will be attended to and will not be withheld from the dog when needed. The ADOPTER agrees to accept complete responsibility for all illness or injury that may occur on the date of adoption of the dog. Even if ADOPTER does not pick up the dog on the adoption day, and the dog is boarded at Raintree Jacks until pick up, ADOPTER is responsible for all veterinary needs.

4. If the dog is microchipped, the microchip will be transferred to the ADOPTER immediately, at the adopter’s expense. Raintree Jacks shall be listed as the secondary owner of the dog with full contact information listed with the microchip company.

5. The ADOPTER agrees to take on all risk associated with being a dog owner, including injury or damage, caused by or in association with the named dog. The ADOPTER understands Raintree Jacks cannot guarantee the future health or temperament of the named dog.

6. Hold harmless and assert no claim, charge, or demand of any kind against Raintree Jacks Inc., or anyone associated with the rescue, for any reason.

7. Adopting party will provide pictures of their yard, fence, where the dog will play, sleep, and of the home and other pets. This will be in lieu of a home visit and pictures must be submitted prior to the application approval.

8. If the adopting party is not local to Raintree Jacks, the adopter is responsible for transporting or arranging transport of the dog to its new home.

9. If the adopting party is not local to Raintree Jacks and transport is required, Raintree Jacks will provide board for the adopted dog for up to 7 days after the adoption agreement is signed at no cost to adopter. Beyond the 7-day period, adopter agrees to pay Raintree Jacks $10.00 per day for board and the care of the dog.

ADOPTER attests that they have read and understand all the conditions and terms of this contract and pledge to abide by them.

ADOPTER understands and agrees to the following stipulations:

- The adopted dog must be given adequate time to adjust to his/her new home,
family and other pets in the home. I, the ADOPTER, agree to separate animals
as needed to slowly introduce the new dog into the home.

- By submitting this application you are making a formal request to adopt a dog from Raintree Jacks, Inc. Filling out an application does not guarantee the dog an applicant is interested in will still be available upon completion and approval of the application. All applicants are thoroughly screened and must meet certain qualifications before
being approved.

- By submitting this application, the ADOPTER states that the information contained within this application is accurate and not misleading in any way.

- Completion of this application authorizes Raintree Jacks Inc. to verify information
contained herein, including verification of medical records of any present or prior pets
treated by the veterinarian listed.

Animals are not a short-term commitment. Make sure your lifestyle allows the time, patience, and financial stability a dog will need over the years.

Adoption contract is entered on this day * between Raintree Jacks, Inc., Cassatt, South Carolina, and * ADOPTER. If ADOPTER does not have a specific dog in mind for adoption and wishes simply to be pre-approved and considered as a future adopter, no fee is required to do so.
