I confirm that my hair is at least to my shoulders, with the shortest piece hitting at least my cheekbone. Photos of your current hair were/must be sent to stylist, as well as inspiration photos prior to appointment date. A phone consultation was/will be had prior to scheduled appointment date between client and stylist, or will be prior to NBR instillation. I am aware that the total price can change if my hair is different in person than what is discussed during the consultation, or if I want/need a change during the appointment. I understand that a $500 deposit is non-refundable and gets applied to service the day of appointment. I understand that Savannah Dawn Hair guarantees the quality of the extension hair used, and will be replaced if quality issue applies. I am aware that if I am entitled to replacement hair due to quality issue, it must be replaced within the first 6-8 weeks of initial install. I am aware that guests traveling back to Savannah Dawn Hair requiring an adjustment within first 8 weeks after the install are responsible for own travel. I acknowledge that “NBR Home Care Maintenance Plan” was/will be discussed in detail with stylist and that a copy was/will be retained for client use. I am aware that client home care is VITAL to optimal results. I am aware that Savannah Dawn Hair does not offer refunds of exchanges on any services. I am aware that all photos taken at Savannah Dawn Hair are property of and can be used for marketing at the discretion of Savannah Dawn Hair and its employees. I will indemnify and hold harmless Savannah Dawn Hair and/or it’s employees free from all loss, damage, claims actions of every kind that they incur or that are claimed or bought against them by any party as a consequence of my use of Natural Beaded Rows hair extensions. I certify that the information I supplied to my stylist during the phone consultation, pre-appointment acknowledgment, appointment acknowledgment, and NBR home care maintenance plan are correct. I agree to indemnify and hold harmless Savannah Dawn Hair and it’s employees for loss, damage, claims or actions of any type that I may suffer as a result of failure to provide them with accurate information.
