We provide payment options as described below. Compliance with our financial policies will minimize the time needed for accounting and billing, thus allowing us to better serve you and your horses.
1. Payment at the time of service-->Check (preferred), cash, or credit card (3% processing fee added for credit card payments)
PayPal, Venmo, or other online vendors are not accepted
2. Credit card information stored securely in QuickBooks and charged following the appointment
This will be required at boarding/training barns in cases where the owner is not present for the appointment, unless other payment arrangements are made in advance with the barn owner/manager.
You are financially responsible for all services provided by Horizon Equine Integrative Medicine. If a balance remains greater than 30 days, a late fee will be assessed.
Declined credit card payments will require immediate alternative payment or action may be taken for collection of the balance due.
Any outstanding balance over 90 days past due will be sent to a collection agency, and you will be responsible for all collection fees and legal fees that our office incurs.
A $30 fee will be assessed for any check returned unpaid by your bank or financial institution.
I HAVE READ AND FULLY UNDERSTAND THE FINANCIAL POLICY SET FORTH BY HORIZON EQUINE INTEGRATIVE MEDICINE, AND I AGREE TO THE TERMS OF THE FINANCIAL POLICY. I ALSO UNDERSTAND AND AGREE THAT THE TERMS OF THIS FINANCIAL POLICY MAY BE AMENDED AT ANY TIME WITHOUT PRIOR NOTIFICATION TO THE CLIENT.
