Screening Prior to Signing Up
You and your dog must receive the recommendation of a GO TEAM approved evaluator. We do contact all evaluators to verify each handler/dog team class recommendation. To schedule an evaluation with an approved evaluator in your area, please exit this form and contact us at training@goteamdogs.org
Service Dog Etiquette Applicants Require Extra Approval to Apply
In addition to screening, All SDE applicants require approval from the GO TEAM Lead Trainer. Contact training@goteamdogs.org before signing up if you have questions.
Consent to use Digital Signature
You will be asked to digitally sign an acknowledgement. If you do not wish to digitally sign the acknowledgement, please exit this form and contact us at training@goteamdogs.org to make alternate arrangements.
Have the following required information ready for upload
- Information about your dog (e.g. age, weight, DOB, fixed?)
- Picture/Scan of your Drivers License (PDF or image, Front Only)
- Picture/Scan of your dogs current Shot Record (PDF or image)
- Picture of your Head Shot for your ID Card (image only, no PDFs)
- Picture of your dog's Head Shot (image only, no PDFs)
If you do not have all 5 items ready for entry/upload, please exit this form now and return when you have them available.
