Van Use Request

This form is for approved drivers with Community Life Bridge. A current background and driver record check as well as a signed Van Use Policy is required for all van drivers. This is also only a request, you will hear from the office if your request is approved, the time the van will be available for pick up, expected back and if a lock box will be necessary.
