    Thank you for Volunteering with Denver Scholarship Foundation (DSF). We are grateful for the time and expertise you will bring to our programs. The following form provides some key acknowledgements for your participation in our program. If you have any specific questions, feel free to contact HR@denverscholarship.org
  • Policies and Operational Procedures 
    I acknowledge and agree that I must comply with all policies and procedures established by DSF. If policies and procedures are not followed it may result in the termination of my volunteer association with DSF. I understand that I or DSF can end this relationship at any time. 

  • Confidentiality Agreement
    I acknowledge and agree that I will preserve the confidentiality of all proprietary and confidential information belonging to DSF or its employees, volunteers and students, including but not limited to employee and volunteer files or student records and information, both while I am providing services to DSF and afterwards. I will not take or misuse any confidential information at any time. I further agree to not use any of this information to benefit myself or any entity, business or person other than DSF.

    Confidentiality Agreement 

    I, the undersigned, understand that during the course of my work, I may be given access to confidential, privileged, or proprietary student information by the District in order to perform my responsibilities in a manner that meets the District’s needs and enhances the delivery of service. By signing this document, I am agreeing to comply with all regulations and laws established to protect confidential information. I understand that accessing or releasing confidential information and/or records or causing this to occur outside the course of my assigned duties would constitute a violation of this agreement. I understand that proven violation of this agreement can result in termination of my access to information and may result in personal action being taken against me.  
     
    “Confidential Information” means any and all information of either party disclosed or otherwise made available to or learned by the parties under this Agreement or performing the Services this Agreement requires, which is designated as “confidential” or “proprietary” or which, under all of the circumstances, ought reasonably to be treated as confidential, and includes, but is not limited to, Student Data and all District student records and personnel records. 
     
    I agree to: 

    • Maintain confidential information and not reveal it to clients, colleagues, or others with whom I interact without procuring the necessary releases or authorizations. 
    • Utilize information disclosed to me solely for the purpose of completing the scope of work set forth in the Data Sharing and Confidentiality Agreement or the Services Agreement. 
  • Property

    I also agree and acknowledge that, on DSF’s request or on termination of my volunteer services, I will promptly return to DSF all its property, including but not limited to all documents, electronic files, or other computer media or other materials in my possession or under my control that contain ideas, processes, concepts or other proprietary or confidential information belonging to DSF or its employees or students.

  • Code of Conduct

    All DSF volunteers are professionals. We are responsible for adding value to the organization and contributing to the success of DSF. We accept professional responsibility for our individual decisions and actions. We are advocates for the organization by engaging in activities that enhance its credibility and value.  
     
    DSF is committed to providing a safe and secure environment for all of our students, Scholars, volunteers and staff. The following areas support our commitment to this safe and secure environment.  

    1. Volunteers will respond to students with respect and consideration and treat all students fairly, regardless of sex, gender identity, race, religion, culture, economic status, or disability.
    2. Volunteers shall take measures to limit closed door time with students.
    3. Volunteers shall not abuse students in any way. No type of abuse will be tolerated and may be cause for immediate dismissal. Types of abuse include:                  •physical abuse---striking, spanking, shaking, slapping, and so on;
      • verbal abuse---humiliating, degrading, threatening, and so on;
      • sexual abuse---touching or speaking inappropriately;
      • mental abuse---shaming, being cruel, and so on;
    4. Smoking or use of tobacco in the presence of students or parents during working hours is prohibited.
    5. Volunteers may not be alone with students they meet in DSF programs outside of DSF program activities. This includes babysitting, sleepovers, driving or riding in cars, and inviting students to their homes.
    6. Volunteers may not date or have sexual relations with any current student or Scholar.
    7. Volunteers are to provide services only within the boundaries of their job role.
    8. Volunteers will not assign a volunteer, student or Scholar to fill in for any staff work assignments.
    9. Volunteers will portray a positive role model for students by maintaining an attitude of loyalty, patience, courtesy, tact, and maturity.
    10. Volunteers are required to report any concerns of abuse, neglect, or intent to hurt self or others to a school official. Volunteers are required to report any concerns of abuse, neglect, or intent to hurt self or others to the on-site DSF staff member. Staff based at a school or college campus must follow its mandatory reporting policies.
    11. Volunteers will act in a caring, honest, respectful, and responsible manner consistent with the mission and values of DSF.
    12. Volunteers are to report any other staff or volunteer who violates any of the policies listed in this Code of Conduct to a manager.

    DSF’s Code of Conduct also applies to all online behavior, whether engaging professionally or personally. Ultimately, you are solely responsible for what you post online. Before creating online content, consider some of the risks and rewards that are involved. Keep in mind that any conduct that (1) adversely affects your job performance, (2) adversely affects or creates a disruption in the performance of fellow employees, (3) creates a disruption or has the potential to create a disruption with clients, suppliers, donors, students/Scholars, stakeholders, other people who work on behalf of DSF, or the general public, (4) has the potential to adversely affect DSF’s legitimate business interests, or (5) violates DSF’s personnel policies set forth herein. Any online behavior that relates to a bona fide occupational requirement, relates to the employment activities and responsibilities of a particular employee or a particular group of  
    employees, or creates either a conflict of interest or the appearance of a conflict of interest, may be grounds for discipline, including immediate termination.  
     
    This Code of Conduct is not exhaustive. Any conduct that DSF deems to be inappropriate or contrary to DSF’s mission and values may result in disciplinary action up to and including termination of volunteer participation. 

  • Photo Release Agreement

    I hereby grant to DSF and to its representatives, employees, agents and assigns the irrevocable and unrestricted right to photograph or record me and use the photo or recording and or other digital reproduction of me or other reproduction of my physical likeness or voice for publication processes, whether electronic, print, digital or electronic publishing via the Internet. I hereby release the DSF and its directors, officers, employees, agents, legal representatives and assigns from any and all claims, actions and liabilities related to use of said photographs or recordings. 

  • Volunteer Release & Liability Agreement

    I acknowledge and agree that activities performed by me as a volunteer will be performed strictly on a voluntary basis, without any compensation, benefits, and workers’ compensation benefits. In consideration of the opportunity to participate as a volunteer for the Denver Scholarship Foundation (DSF), I, release and forever discharge and hold harmless DSF, its directors, employees, agents, successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the services I provide to DSF. I understand and acknowledge that this Release discharges (DSF) from any liability or claim that I may have against (DSF) with respect to bodily or, personal injury, illness, death, or property damage that may result from the services I provide to DSF or occurring while I am providing volunteer services.
