Swords to Plowshares Media Consent Form

We are grateful when veterans, volunteers, donors, and community members choose to share their experiences to inspire support for our work, represent our mission, and increase public awareness of veterans’ struggles, strengths, and livelihoods. 

Media and storytelling participation is voluntary. Choosing not to participate will not affect the ability to receive services or support from Swords to Plowshares.

By signing this form, I understand that photographs, video recordings, audio recordings, interviews, quotes, stories, and other information I choose to share may be used in the ways authorized on this form and may be viewed, read, heard, or shared by the public. 

I understand that once materials are published, distributed, broadcast, shared online, or provided to members of the media, Swords to Plowshares may not be able to control how those materials are further shared, reproduced, or used by others. 

I understand that Swords to Plowshares may edit materials for length, clarity, formatting, or presentation while preserving the overall meaning and intent of my story. 

I voluntarily grant Swords to Plowshares permission to use the materials I have authorized above and release Swords to Plowshares from any claims arising from the authorized use of those materials, including claims related to privacy, publicity, defamation, or copyright. 

I understand that participation is voluntary, that I will not receive compensation for the use of these materials unless otherwise agreed in writing, and that choosing not to participate will not affect my ability to receive services or support from Swords to Plowshares.
