1. 本人已年滿18歲。I have attained the age of 18 years.
2. 本人聲明在此報名表格及隨附文件所載的資料，依本人所知均屬完整真確。所發之證書以表格上英文姓名為實，如有更改需另付費用。I declare that all information in this application form and the attached documents are, to the best of my knowledge, accurate and complete. Applicant name shown on the printed certificate will be same as the name he/she has provided. Handling charge will be applied for each requested change.
3. 本人同意如本人獲取錄入學，本人當遵守學院的規例。I consent that if admitted, I will comply with all the Rules and Regulations stipulated by the Institute.
4. 本人已細閱報名表格內之申請須知，並明白所有報名須知詳情均以本院課程手冊及網頁之最後更新版本為依歸。I have read the “Notes for Application” in this application form and understand that the details of the Enrollment Guidelines are subject to revisions in the Institute prospectus and the latest updates in the Institute website.
5. 已繳付學費或訂金均不能退還。No deposit or school fee paid will be refunded.
6. 於外部質素保證過程中，TQUK / TQUK ESEA 將索取報讀學員之個人資料，亦會委派代表進行觀課 (實體課和網課，如需要時)。For an external quality assurance, TQUK / TQUK ESEA will access applicant’s personal data and sit-in regular lessons (face-to-face lessons and zoom lessons, when needed).
7. 學員須於報讀課程當天起計算六個月之內完成相關課程之標準學習時數。Applicants are required to complete his/her learning hours within 6 months, starting from the day of course registration.
8.如學員對上述條款有任何爭議，本學院將保留最終決定權。In case of disputes, our institute reserves the rights of final decision.
個人資料收集聲明及媒體及私隱政策 Statement on Collection of Personal Data & Media and Privacy Policy
本學院收集及保存學員個人資料的目的及用途為審核培訓課程的申請、安排入讀事宜、提供就業跟進、安排課程接受評審、郵寄章程，以及作就業記錄覆核或意見調查等。有關資料或會被轉移至相關僱主及╱或政府部門及╱或其他相關部門作上述的用途。
學員參與的所有課堂及活動，都可能會被學院拍攝照片及錄影，亦有機會被上載至官方媒體（例如學院網站、社交媒體及任何宣傳材料）。若學員不希望其影像在上述媒體上出現，請於報名時明確通知學院。
學院在處理所有個人資料時，將遵守《個人資料（私隱）條例》的所有相關規定，並確保所有資料以安全及保密的方式加以管理。
The institute collects and keeps the personal data of applicants for the purpose of reviewing applications for training courses, arranging admission matters, providing employment follow-up, arranging for courses to be reviewed, mailing regulations, and conducting employment record review or opinon surveys, etc. The information may be transferred to the relevent employers and/or government departments and/or other relevant departments for the above-mentioned purposes.
The institute reserves the right to capture photographs and/or record short video clips during classes and related activities for the purpose of documentation and dissemination through official channels. Participants who do not wish their image or likeness to appear in any media must notify the institute at the time of registration. Failure to do so will be regarded as acceptance of the above terms.
The institute is committed to safeguarding the privacy and confidentiality of all personal data collected. All personal information will be handled in strict compliance with the Personal Data (Privacy) Ordinance (Cap. 486) and any other applicable laws and regulations.
