Privacy Notice, Data Protection and Specimen Collection Disclaimer

1) I give my consent for phlebotomy (blood draw) and collection of other specimens for the purpose of laboratory evaluation of certain physiologic markers, determined by the physician(s) or by self referral, which may be used for medical evaluations and diagnosis of health.

2) The original of this consent will be stored for minimum one month after final report release. Pathology results, specimens and personal data will be stored according to appropriate regulations/company policy. Report will be released only to recipients, including doctors and/or practice, specified on request form, by default via e-mail.

3) I agree to use my details for purpose of physiologic markers evaluation, to send via e-mail and/or Royal Mail pathology results and newsletters, and to contact me by Isac Labs Ltd, if necessary. Consent and personal details will not be released to any individual, third party, or other requestor without expressed written consent to release this information, unless it’s a legal requirement, e.g. notifiable organisms that must be reported Public Health England.

4) I understand and agree that Medical Diagnosis Ltd may be required to transfer its patient’s specimens to the third parties – referral laboratories, to carry out certain requested tests.

5) I know and understand my right to be forgotten. Erase process must be followed by written consent and is irreversible.

6) I know and understand my right to access and rectification of personal data. Data will be rectificated automatically according to request form. Rectification process is traceable.

7) I know and understand my right to the restriction or suppression of personal data.

8) I know and understand my right to object to the processing of personal data in certain circumstances.

9) I know and understand my right to data portability.

10) I know and understand my rights related to automated decision making including profiling.

11) If the patient is unconscious or unable to give informed consent, including children below the age of 16, the next of kin or legal guardian (which can be a court of law) can give permission for a specimen to be taken and process of personal data.

12) I know and understand the patient’s rights and risk related to specimen collection.
