Health check bookings are available at selected locations and are subject to demand and availability. 

 

Terms and Conditions

a.	Members aged 16 or over will be entitled to one annual health check per claims year.
b.	MedPro services are provided by registered nurses from Southern Cross Wellness Partners Limited.
c.	Health check appointments are available at selected locations and are subject to demand and availability.
d.	If the Member does not attend their appointment for an Annual Health Check or the Member provides MedPro with less than two business days’ notice of cancellation of their appointment, the missed appointment will be counted as the Member’s Annual Health Check for that claims year.
e.	MedPro will have access to limited information about you in Southern Cross’ system (such as your name, contact details, your Southern Cross member identifier) to help validate and book your health check. Southern Cross will use the record this creates to process and analyse health check uptake. Southern Cross won’t receive any health information from your MedPro check.
f.	The policyholder of the Southern Cross Health Insurance policy you book under will be notified once you have had your annual health check with MedPro. If you aren’t comfortable with the policyholder knowing you are having this health check, please don’t continue with this form.
