This form does not guarantee your appointment date and time and is required for both styling appointments and hair pieces. Once this form is submitted, you will be contacted for a consultation in which your appointment and deposit amount ($10-$50) will be confirmed. Your deposit amount will be determined by the amount of braiding hair needed for the style you choose and will be applied towards your grand total. Deposit is required to book appointment.
