• AMP Vendor Form

    Signing up does not guarantee a table. You will be contacted to confirm your vendor table.

  • Complete this form to be a vendor at an Ascend through Music special event or program! As a vendor, you will be able to sell your products and services to people attending the event.

     

    Each vendor will be provided one table and 2 chairs. You must bring your own table decorations and display. Additional expectations for each specific event will below. 

     

    Vendors must pay before setting up for their event. Vendors can pay via CashApp ($ascendmp) or cash.
