For your convenience we accept cash, checks, Mastercard, Visa, Discover, American Express, Debit Card, Care Credit, and Scratch Pay. Payment is due when services are rendered.
I assume responsibility for all charges incurred during the handling, or medical and/or surgical treatment of any animals contained in my permanent record. If I do not pay for services, I understand that I will be charged interest on any overdue balances. A FINANCE CHARGE OF TWO PERCENT (2%) PER MONTH (24% ANNUAL PERCENTAGE RATE), WITH FIFTY CENTS ($.50) MINIMUM CHARGED ON THE UNPAID BALANCE. AT THE OPTION OF THE HOSPITAL, THE ENTIRE BALANCE MAY BECOME DUE AT ANYTIME. If it is necessary for Ashby Animal Clinic to turn this account over to a collections attorney for nonpayment, I will be responsible for all costs of collection, including, without limitation, attorney's fees and court costs.
