Please use this portal to upload your Proof of Completion PDF. You do not need to upload all certificates at one time.
- Click Save and Continue Later and the form will email you a custom link to return to this page to add new documents as you complete your courses.
- Once you have uploaded all required documents, click Submit Documents to finalize your submission.
- If you have an issue uploading a file, please rename the file by appending the year to the filename (e.g. LAST_FIRST_YEAR.pdf).
