Subject: Authorization to Obtain Credit Reports
I hereby authorize DLS Consulting Firm and Daimon L. Sweet, M.Ed., Credit Restoration Consulting Specialist, to obtain copies of my consumer credit reports from the three major credit reporting agencies: Experian, Equifax, and TransUnion.
This authorization grants DLS Consulting Firm permission to access my credit reports solely for the purpose of providing credit restoration consulting services, including but not limited to reviewing, identifying, disputing, and working toward the removal of inaccurate, unverifiable, outdated, or otherwise inadequate information from my credit file.
I understand that:
This authorization does not permit the release of my credit information to any third parties other than for the purpose of providing consulting services related to my credit restoration.
This authorization will remain valid until the completion of services, or until I provide written notice to revoke authorization.
I may request a copy of my credit reports obtained and reviewed under this authorization.
By signing below, I acknowledge that I have read and fully understand this authorization and grant DLS Consulting Firm the right to obtain and use my credit reports as outlined above.
