By signing this form, I understand that once any package(s) and/or article(s) of letter mail is/are released to the person listed as a "proxy" on this form, I am releasing Residential Life and Housing from any and all liability regarding the delivery of the listed items(s) and that I am responsible for coordinating with and retrieving my items(s) from said person.
Instructions
DO NOT complete this form until a package or letter has been delivered and you have received a notification email from RLH Mail Services that the item is ready to be picked up. Requests submitted after 5:00pm may not be processed until after 9:00 am on the following business day. All requests made during business hours can take up to four hours to process from the time of submission. This form MAY NOT be used by former on-campus residents. All packages and letters that are received for residents that no longer reside on-campus will be returned to the original sender. Thank you!
