A criminal background check will be required for all students.
The following health documentation must be completed by all students (if applicable)
All students must wear scrubs during clinical rotations.
A 90% attendance rate is mandatory for the entire course, with tardiness being counted as absences.
A passing average of 75% is required for successful course completion.

Social Media and Cell Phone Policy:
- Posting or blogging on any type of internet networking site is not permitted during class time or clinical time.
- Posting or blogging any information regarding a patient’s identity or condition, patient care, patient treatment plans – any information about a patient in any form, via electronic, hard copy, or verbal, is on a “need to know basis” per HIPAA regulations.
- As healthcare students, HIPAA rules must be adhered to at all times, even times you are away from campus or clinical.
- HIPAA violations may be federally prosecuted, and failure to comply with HIPAA regulations is grounds for dismissal from this institute.
- Cell phones are not allowed in class or at any clinical site. Do not bring your cell phone to class or clinical.
- Please give your instructor your emergency contact information prior to the start of your class and/or clinical rotation in the event of an emergency.
- Any violation may result in dismissal from this institute.

Program Application and Background Authorization
Authorization Statement:
I understand that as part of the enrollment process, I will be subject to a Background Check. I understand that there will be a non-refundable fee included in the tuition to cover this cost. I understand and agree that if my record shows a felony, charges indicating violence, fraud, abuse, and/or charges that make me ineligible for clinical training, I will not be allowed to participate in the programs I have applied for and am NOT due a refund.
I authorize the Persevere Allied Health Institute to run this check or to access a check completed within 12 months by my employer or another local or state agency.

Declaration of Citizenship or Legal Presence in the United States
By signing this statement, you attest that you are a US citizen, a legal permanent resident in the United States, or an alien lawfully present in the United States. In addition, the Persevere Allied Health Institute may require you to submit documentation that supports your claim.
Any student providing false information may be subject to dismissal from the Persevere Allied Health Institute. Any student who is found to be unlawfully present in the United States will be dismissed from the Institute.

Program Policies & Procedures Student Certification:
I acknowledge that I have received a copy of the student handbook and that I have read, understand, and will abide by the policies and procedures in the Student Handbook.
Policies and procedures mentioned in the handbook include, but are not limited to, those related to attendance, class participation, academic honesty, and the use of institute resources. I understand that violating any of these policies may result in disciplinary action, up to and including expulsion from the institute.
PLEASE NOTE: Some of these rules will not apply to ALL programs. (Ex. The IV program does NOT include a grading system associated with it.)

Hepatitis Waiver
In compliance with OSHA Regulation: 29CFR BLOODBORNE PATHOGENS STANDARD 1910.1030: the student is advised that OSHA recommends persons at substantial risk for HBV (hepatitis B) should be vaccinated. Individuals are often at substantial risk for HBV if they are exposed to blood or other potentially infectious materials through their employment. The vaccination is available at the Student Health Service, the costs of which will be borne by the student.
If the student has already been vaccinated for HBV and provides evidence of such, he/she may sign a waiver declining the vaccine.
If the student has not been vaccinated and declines the vaccination, he/she must sign a waiver acknowledging:
1) The risks associated with HBV; and
2) That the decision to decline the vaccine was made after being informed of the risks and benefits of the vaccine.
3) The student will assume all responsibility for any injury or illness that may result from exposure to HBV.
Persevere Allied Health Institute Hepatitis B Vaccination Waiver
I have been advised that I may be at substantial risk for contracting HBV (hepatitis B) through my employment in the healthcare field. I have also been informed of the benefits of the HBV vaccine and have had the opportunity to ask questions about the vaccine and its risks and benefits.
I have chosen not to receive the HBV vaccine at this time. I understand that by declining the vaccine, I remain at risk for contracting HBV if exposed to blood or other potentially infectious materials through my employment. I also understand that if I do contract HBV, I may suffer serious health consequences, including liver damage, cirrhosis, liver cancer, and even death.
I hereby release Persevere Allied Health Institute from any and all liability for any injury or illness that I may suffer as a result of my exposure to HBV. I also agree to hold harmless Persevere Allied Health Institute, its agents, and employees from any and all liabilities or claims that may arise from my decision to decline the HBV vaccine.

Policy Notification Regarding Health Status Change:
1. Any student experiencing a change in health status while enrolled will be required to submit a Health Change Status form from his/her physician or nurse practitioner.
2. Pregnant students must submit a Change in Health Status form from their healthcare provider before registering each semester.
3. The Change in Health Status form/Withdrawl form must be obtained from the Managing Director or Program Administrator.
4. The student must submit these documents prior to continuing her clinical experience.
5. Specific programs may require additional documentation.
6. I hereby certify that the information on this form is correct. I understand that false information will be sufficient cause for the college to cancel my enrollment and require withdrawal.
7. I will report any changes in my health status to the Division of Health Sciences and submit a completed change in health status form.
8. I authorize the Division of Health Sciences to release this information to the agencies where I have clinical assignments.

Venipuncture Participant Release and Indemnification Agreement
I the undersigned student of Persevere Allied Health Institute:
UNDERSTAND AND HEREBY EXPRESSLY ACKNOWLEDGE that as part of the instruction I am to receive as part of the PHLEBOTOMY PROGRAM, I will be asked to draw blood by venipuncture or finger stick on other Program students and that such other Program students will be asked to practice drawing blood by venipuncture or by finger stick on me.
UNDERSTAND AND HEREBY AGREE that accidental exposure to human blood or other potentially infectious materials (OPIM) must be reported immediately. I understand that I will be expected to obtain a risk evaluation conducted by a clinician familiar with post-exposure evaluation and treatment, which is recommended by the Centers for Disease Control and Prevention (CDC), and if deemed necessary, initiation of post-exposure prophylaxis (PEP). The CDC specifically recommends that PEP be initiated within two hours of HIV exposure to prevent disease transmission. I understand that I am personally responsible for the cost of the post-exposure medical management and treatment and that Technical College of the Lowcountry is in no way responsible for these expenses.
HEREBY AGREE to follow Standard Precautions guidelines as well as to comply with regulations outlined in the OSHA Bloodborne Pathogen Standard in order to minimize the risk of exposure to bloodborne pathogens.
HEREBY RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Persevere Allied Health Institute, its officers, agents, servants, employees, assigns, or successors from any and all liability, claims, demands, actions, or causes of action arising out of any damage, loss or injury resulting to my person or my property or resulting in my death while enrolled in Persevere Allied Health Institute Phlebotomy Program and participating in the activities contemplated by this release.
HEREBY REPRESENT that I am physically able, with or without accommodation, to participate in the above-referenced activities, and I understand that it is strongly recommended that each participant in the Phlebotomy Program purchase insurance that covers accidents that may occur during participation in the Phlebotomy Program activities.

Program Policies & Procedures
Admission Requirements
To be eligible for admission into the PAHI Phlebotomy Technician program, applicants must:
Be at least 18 years of age
Have a high school diploma or equivalent
Submit a completed application form
Pass a criminal background check
Application Process
Complete and submit the online application. Pass a criminal background check.
Upon review of your application, you will be contacted to schedule a mini interview. During the interview, you will have an opportunity to ask questions and learn more about the program.
Academic Information
The PAHI Phlebotomy Technician program is a 4-week, full-time program. The curriculum is designed to provide students with the knowledge and skills necessary to succeed as a phlebotomy technician. Students will receive both classroom and laboratory instruction, as well as clinical experience in a variety of healthcare settings.
Classroom instruction will cover topics such as:
Phlebotomy techniques
Specimen collection and processing
Quality assurance and safety procedures
Clinical rotations will provide students with the opportunity to apply their knowledge and skills in a real-world setting. Students will rotate through a variety of clinical sites, including hospitals, clinics, and blood banks. Upon successful completion of the program, students will be eligible to take the national certification exam.
Attendance Information
Regular and punctual attendance is essential for success in the PAHI Phlebotomy Technician program. Students are expected to attend all scheduled class sessions and clinical rotations. If a student must miss a class or clinical rotation, it is the responsibility of the student to notify their instructor or preceptor as soon as possible. Excused absences may be granted for illness, family emergencies, or other extenuating circumstances. Unexcused absences will result in dismissal from the program.
PLEASE NOTE: Because we do understand that incidents occur, we will allow up to ONE reschedule for the same class/course that you originally scheduled for. Reschedule must occur within 30days of your originally scheduled class/course & admitted for the cohort within 3 months following your withdrawal approval.
Tardiness: Students are expected to arrive on time for all scheduled class sessions and clinical rotations. If a student is more than 15 minutes late, it will be counted as an absence.
Illness: If a student is ill and cannot attend class or clinical, they must notify their instructor or preceptor as soon as possible. A doctor's note may be required.
Family Emergency: If a student has a family emergency, they must notify their instructor or preceptor as soon as possible. Documentation may be required.
Extenuating Circumstances: If there are extenuating circumstances that prevent a student from attending class or clinical, they must notify their instructor or preceptor as soon as possible. Documentation may be required.
Jury Duty: If a student is summoned for jury duty, they must notify their instructor or preceptor as soon as possible. Documentation may be required.
Inclement Weather Policy: In the event of inclement weather, please check the PAHI website or call PAHI at 313.473.0434 for information on class cancellations. All students will receive an email with school closing information from Perseverealliedhealth@gmail.com.
Class Cancellation Policy
The PAHI Phlebotomy Technician program reserves the right to cancel any class due to low enrollment. If a class is canceled, students will be notified as soon as possible and will be given the option to transfer to another class or receive a full refund.
Coursework & Grading
The PAHI Phlebotomy Technician program is graded on a A—F grading system.
GRADING SYSTEM:
1. Uniform- 10pts per day and 11pts on the last day equating to 81pts.
Must have all parts of uniform in order to receive the points. NO PARTIAL POINTS.
-Correct uniform: Watch, neat and clean scrubs, and white clean shoes
2. Lab Attendance- 10pts per day and 11pts on the last day equating to 81pts.
Must be present within the 15min grace period to receive FULL points for that day.
3. Portal Attendance- students must be signed-in for a total of 72 hours for the full program term (18 hours per week) which equates to 360 total points. Students are to complete all module work outlined for each day and get a 75% or higher on all quizzes in order to receive FULL 30 point credit for each course day. All or nothing point system.
4. 3 Practice exams: 85pts per exam = 255pts
5. Extra credit: 3pts per worksheet = 123pts
Students have the potential to earn up to 900 points total.
Upon successful completion of the program and a 75% overall grade, students will receive a certificate of completion.

Clinical Rotations/In class Skills lab practice will occur 2 times per week on Wednesdays and Fridays. (More time can be provided upon a scheduled request)
Upon successful completion of the program, students will be eligible to take the national certification exam through National Healthcareer Association.
Certification
Students who successfully complete the PAHI Phlebotomy Technician program will be eligible to take the national certification exam. Upon passing the exam, students will be awarded the designation of Certified Phlebotomy Technician (CPT).

Class Schedule
The PAHI Phlebotomy Technician program is offered in hybrid format only. The class schedule is as follows: Monday, Tuesday and, Thursday, 9:00am–3:00pm, at home computer course work. Wednesday and Friday 10:00am-2:00pm OR 3:00pm-7:00pm, in class skills/clinical practice.
Clinical Rotations & Externships
All students in the PAHI Phlebotomy Technician program are required to complete a minimum of 32 hours of clinical rotations/skills lab.
Coursework
The PAHI Phlebotomy Technician program curriculum is designed to prepare students for a career in phlebotomy. The coursework includes both didactic and clinical components. Upon successful completion of the program, students will be eligible to take the national certification exam.
Tuition & Fees
The tuition for the PAHI Phlebotomy Technician program is $2,222, which includes all tools, lab supplies, uniforms, in-class exams and quizzes and the National exam. Tuition and fees are NON-REFUNDABLE. Because we do understand that incidents occur, we will allow up to ONE reschedule for the same class/course that you originally signed up for. You must give us a new start date within 30 days of your originally scheduled class/course. The new start date must be within a 365 day period from your original start date.
Financial Assistance
The PAHI Phlebotomy Technician program is not eligible for traditional financial aid. Students who wish to apply for financial assistance can do so by applying for a voucher. For more information on the voucher programs in our area, please contact Michigan Works! Or #211 for help.
Course Withdrawal Policy
Students may withdraw from a course at any time up until the withdrawal deadline, which is 2 weeks prior to the first day of class. To withdraw from a course, students must complete a Course Withdrawal form and submit it to the PAHI office. A grade of "W" will be assigned for each course from which a student withdraws. Please note that withdrawing from a course may affect a student's financial aid eligibility.
Academic Probation Policy
Students who receive a grade of "C" or below in any course will be placed on academic probation. Students on academic probation will be required to meet with their academic advisor to develop a plan for success. Students who receive a grade of "C" or below in two courses will be automatically dismissed from the program.
Dismissal Policy
The PAHI Phlebotomy Technician program is a 4 week, full-time program. Students are expected to attend all scheduled class sessions and clinical rotations/skills lab. Professional Conduct
Confidentiality Policy: All information pertaining to patients, their families, and other healthcare workers must be kept confidential. Students who violate this policy will be subject to disciplinary action, up to and including dismissal from the program.
Confidential information includes but is not limited to:
· Patient names
· Patient diagnoses
· Patient treatments
· Patient prognosis
· Patient medications
· Patient test results
HIPAA Policy: The Health Insurance Portability and Accountability Act (HIPAA) is a federal law that protects the confidentiality of patient health information. All students in the PAHI Phlebotomy Technician program must complete the HIPAA training module prior to beginning their clinical rotations. Students who violate the HIPAA policy will be subject to disciplinary action, up to and including dismissal from the program.
Harassment Policy: PAHI does not tolerate any form of harassment or discrimination. Any student who engages in harassing or discriminatory behavior will be subject to disciplinary action, up to and including dismissal from the program.
Code of Ethics: All students in the PAHI Phlebotomy Technician program are expected to abide by the Code of Ethics of the American Society of Phlebotomy Technicians. A copy of the Code of Ethics is available here.
Guidelines for Appropriate Use of Social Networking Websites: Social networking websites such as Facebook, Twitter, and Instagram can be valuable tools for networking and professional development. However, students are reminded that these websites are public forums and should be used accordingly. Students are advised to use caution when posting information on social networking websites and to refrain from posting any confidential or personal information. In addition, students should avoid making any negative or disparaging comments about PAHI, its faculty, staff, or students. Students who violate this policy will be subject to disciplinary action.
Personal Privacy: The PAHI Phlebotomy Technician program respects the personal privacy of its students. All student records are confidential and will not be released without the student's written consent, except as required by law.
Alcohol and Drug Policy: The use of alcohol and drugs is strictly prohibited on PAHI property or at any PAHI-sponsored event. Students who violate this policy will be subject to disciplinary action, up to and including dismissal from the program.
Protection of Patient Information: The PAHI Phlebotomy Technician program is committed to protecting the confidentiality of patient information. All students in the program must complete the HIPAA training module prior to beginning their clinical rotations. Students who violate the HIPAA policy will be subject to disciplinary action, up to and including dismissal from the program.
Violence Prevention Policy: PAHI does not tolerate any form of violence or threatening behavior. Any student who engages in violent or threatening behavior will be subject to disciplinary action, up to and including dismissal from the program. Please note that these policies are subject to change at any time at the discretion of PAHI.
Professionalism: All students in the PAHI Phlebotomy Technician program are expected to conduct themselves in a professional manner at all times. Professionalism includes, but is not limited to, exhibiting courtesy, respect, and sensitivity; maintaining confidentiality; adhering to the dress code; and abiding by the policies and procedures of PAHI and clinical sites. Students who violate the professionalism policy will be subject to disciplinary action, up to and including dismissal from the program.
Dress Code: All students in the PAHI Phlebotomy Technician program are expected to dress in a professional manner at all times. Professional dress includes, but is not limited to, scrubs, a white lab coat, and closed-toe shoes. Students who violate the dress code will be subject to disciplinary action.
Conviction of a Crime: Any student who is convicted of a crime while enrolled in the PAHI Phlebotomy Technician program will be subject to disciplinary action, up to and including dismissal from the program.
Failure to Comply with Program Policies: Any student who fails to comply with the policies of the PAHI Phlebotomy Technician program will be subject to disciplinary action, up to and including dismissal from the program. These policies are subject to change at any time at the discretion of PAHI.
Discrimination and Harassment Policy: It is the policy of PAHI that all students, faculty, and staff be treated with respect and dignity. Discrimination or harassment of any kind is strictly prohibited and will not be tolerated. Students who violate this policy will be subject to disciplinary action, up to and including dismissal from the program.
