THE PARENT OR GUARDIAN MUST COMPLETE THIS FORM FOR EACH MINOR ATTENDING CAMP. 

LEADERS/CHAPERONES ATTENDING CAMP MUST ALSO COMPLETE A FORM FOR THEMSELVES.

We want the most current medical information.

Please complete form NO EARLIER than 2 weeks before camp.

 Medication Requirements

1. ALL meciation must be listed here in order for us to dispense.

2. Prescription medication must be in the original container with the camper's name on the label and include physician's instructions.

3. All non prescription medications or vitamins must be in the original pacakging and will be dispensed according to package directions.

4. We keep a supply of the typical over-the-counter medications on hand to dispense should your child need care. (Tylenol, Ibuprofen, Tums, Pepto Bismol, Cough Syrup, and Benedryl).

5. All medication should be packed separetly and given to your group leader so that it can be checked in at registration EXCEPT for Epi-Pens and Emergency Inhalers.

6. Campers are encouraged to keep those two items in their possession. Please clearly mark them with a permanent marker including the camper's name and group on the container as campers often misplace such items.

List the name of another person who can be contacted in the event of an emergency or illness and you cannot be reached. 

Any person listed should be over 18, able to assist in contacting you, and be able to drive up to camp.

Parents cannot be listed again as emergency contacts for minor campers.

We will alway seek to contact parents first.

Camp can be a pivitol time in a child's life. You can suprise your kid with a gift card for the camp store!

Or help another under resourced kid come to camp!

 

PLEASE NOTE: Gift cards are nonrefundable. Camp Pondo is not responsible for lost or stolen gift cards. We cannot refund unused money left on a gift card. However, gift cards do not expire and can be used in future seasons. The Pondo gift and snack shop accepts cash, debit, and credit cards as well as gift cards.
