The Essence of You - Holistic Therapy and Coaching provides confidential and professional support to all clients who engage with our services.
Any information shared with us is considered confidential. We cannot disclose the information to anyone without us obtaining consent from you in written form.
In accordance with professional responsibilities and/or legislation, we may need to disclose details to other external persons in the following circumstances:
Exceptions to Confidentiality:
- If you threaten harm to yourself or to another person
If we believe a child or protected adult is at risk of harm or abuse
If the courts instruct us to give information
If you share information about a proposed act of terrorism or other illegal act
If we feel that either you or someone else is in danger or at risk of harm I would first endeavour to discuss with you my decision for breaking confidentiality.
However, I retain the right to break confidentiality without prior consultation with you should I consider that the urgency of the situation requires me to act immediately to safeguard the physical safety of yourself or others.
We maintain individual client files. This will contain records of any contact, key discussion points, outcomes and other relevant information provided during consultation with you. Your client file is kept in a secured folder and your information is not shared with any members of the public or other professionals unless agreed to by you in writing.
We retain client files for a minimum period of 15 years after completion.
Access to your client file may be requested by you in writing.
You will be asked to sign a Receipt of Personal Case Notes form when the information is provided to you.
