• 2022 ACMS Stipend Request Form for IMA House of Delegates

  • Thank you for serving as an ACMS delegate to the Idaho Medical Association House of Delegates. I hope the experience was informative, inspiring, and useful for you. Your allowable stipend and/or travel reimbursement amount is listed below based on your selections above. 

     

  • ACMS offers the choice of donating a portion or all of your stipend to the ACMS Foundation funds listed below. Also, because of the high number of ACMS delegates that signed up this year, we will award stipends/reimbursements based first on category priority and then based on registration date thereafter. 

    1) All ACMS members in the order of registration date

    2) Representative of an educational institution in the order of registration date

    3) Representative of a medical specialty society in the order of registration date

    4) IMA Board of Trustees Member in the order of registration date

    If you are being reimbursed for travel expenses by another organization/institution, we respectfully ask for you to decline the stipend/reimbursement altogether this year.

  • Please use a redirect amount that does not make the total stipend to calculate to less than $0. (Use positive, whole numbers only and no $ symbol).

  • All stipends are considered taxable income, unless you claim it as volunteer travel expenses on your tax return and retain documentation for the IRS.

    Since your amount is $600 or greater, ACMS will necessarily issue you an IRS 1099-Non Employee Compensation form as reportable taxable income. You can avoid this by providing ACMS copies of travel expenses for meals, lodging, mileage and we will issue the appropriate portion as a reimbursement rather than a stipend. Please upload your receipts and state the travel amount in order to accomplish this.

    Also, we will need you to fill out and submit a copy of this form.
