ETHAP Foster Application

Thank you for your interest in fostering for East Texas Hoof and Paw Animal Rescue (further abbreviated as “ETHAP”). We take pride in caring for our animals and hope to find fosters who care just as much as we do! We look forward to learning more about you. Please read our general requirements before submitting a foster application. Thank you!

Applications must be filled out completely and truthfully. Applications that are not filled out completely will not be processed.
All applicants must be 18 years of age or older.
We do not place our pets on a first come, first serve basis, but rather place them in the home that is the best fit for the pet.
All current pet(s) in the home must be up to date on age appropriate vaccinations, and spayed/neutered if 6 months or older. If a pet does not meet these qualifications due to a medical reason please specify.
Please be aware of any rental/HOA/city ordinance restrictions such as breed/weight restrictions, number of pets allowed, etc.

I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any ETHAP animal fostered by me.

I understand that by submitting this questionnaire, I agree to release and covenant to hold harmless ETHAP and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.
I accept full responsibility for the dog(s) actions at all times, and release ETHAP from any liabilities or damages that may be incurred because of fostering such dog(s).
I agree to have ETHAP complete reference call checks and potentially conduct a home visit to be able to approve my foster application. Along with potential random home checks if ETHAP deems necessary.
I agree that if I'm unable to foster the dog(s) anymore that I will notify ETHAP asap and do my best to allow ETHAP 14 days to find other arrangements for my foster dog(s) while still keeping my foster dog(s) safe with proper care.
