- Collection will be processed upon receiving completed form with fully paid orders upon walk-in to EC.
- Each person is allowed to submit a maximum of 3 orders for collection per visit.
- Kindly perform thorough checking of item(s) and verification upon collection. Young Living Malaysia shall not be liable for any discrepancies once collection process is completed
- Upon submission of JotForm, no changes or request to retract submission will be entertained via phone, email or chat.
- All submissions, especially if submitted by a third party, will be deemed to have been submitted with the consent or authorization of the account holder.
- Young Living shall not be held liable for any loss/damage due to negligence of members.
- I verify and affirm that all information provided is accurate and correct.
