1:1 Chromebook Optional Insurance Plan Sign Up

Lakeview Public Schools is offering families the option to purchase a technology insurance plan for their district-provided Chromebooks and accessories. The insurance must be purchased each year and will cover accidental damage to the equipment as well as theft from school property as outlined below.
Families who choose NOT to purchase the optional insurance will be responsible for all costs required for repairs or replacement of the district-provided equipment.

Basic Insurance Plan - $25/Year/Child (Family Maximum is $75/Year)

Annual Deductibles
- 1st repair: Free
- 2nd repair: $25
- 3rd repair: $50
After the third repair, a meeting between the parents and the building principal will take place to discuss the issues and what to do moving forward.

Lost Chromebooks:
- Chromebooks are the responsibility of the student; loss is not covered by the district’s optional insurance plan.
Stolen Chromebooks:
- Chromebooks stolen at school may be covered once verified by a school investigation.
- Chromebooks stolen off school property are the responsibility of the student and are not covered by the district’s optional insurance plan. Families have the option to claim the loss through their personal insurance.
To sign up for this optional insurance plan, please fill out the information below for your JMS and/or LHS student(s). You will then need to sign in to your child's Lakeview EZPay account to purchase the insurance for EACH of your grades 6-12 students for the year.

Your child's insurance coverage will start once your have submitted you payment in full for each child through EZPay.

* If you have more than 3 JMS/LHS students, you do not have to pay more than $75 for family insurance coverage. Please include all of their names above to sign them up, but you only have to go into EZPay for 3 of your students to pay the $25 each. We will cross reference on our end to ensure all of your students are covered.
